Are premium multifocal lenses covered by health insurance?

I am 58 and the doctor suggested multifocal lens over monofocal for long-term benefit. Will insurance cover any part of the lens cost, or is it entirely out-of-pocket for premium lenses?

Most insurance plans in India do not cover premium multifocal lenses; coverage usually applies only to standard cataract surgery with monofocal lenses. Patients typically pay extra out-of-pocket for multifocal or toric lenses.
