I have bilateral mild varicocele, sometimes shown as varicocele grade 2 in reports, and I’m worried about reduced sperm count, too. How to increase sperm count with a varicocele medically? Does my symptom require microscopic varicocele surgery? Also, is a hydrocele different than a varicocele, or are they related conditions?

As a doctor, I want to clarify a few important points. Bilateral mild varicocele (grade 1–2) can sometimes affect sperm count, but many men remain fertile. In some cases, lifestyle measures, like maintaining a healthy weight, avoiding heat exposure, reducing smoking and alcohol, may help optimize sperm health, but they do not treat the varicocele itself.
Hydrocele is a different medical condition, and refers to the fluid around the testicle, whereas a varicocele is enlarged veins in the scrotum. Microscopic varicocele surgery is usually considered only if the varicocele is causing significant symptoms, pain, or fertility issues.
