Bleeding is a normal part of the medical abortion process. For most people, the bleeding is heaviest within the first 3–6 hours after taking misoprostol (the second pill), when the body expels the pregnancy tissue. This heavy bleeding usually includes clots and strong cramps, which is expected.

After the initial heavy phase, bleeding gradually decreases and may continue like a lighter period or spotting for about 1–2 weeks. Some may experience intermittent spotting for up to 4 weeks, which can still be normal.
