I suffer from PCOS and I often experience heavy bleeding with severe cramps during my periods, sometimes lasting more than 10 days. Is this normal with PCOS or could it be a sign of some other problem like endometriosis? How do doctors differentiate between these conditions?

Heavy bleeding can happen with PCOS because irregular ovulation allows the uterine lining to build up, but periods that last more than 10 days with very strong cramps are not typical of PCOS alone. Conditions like fibroids, endometriosis, or thyroid problems can also cause these symptoms.
To tell the difference, doctors usually start with an ultrasound to check the uterus and ovaries. If endometriosis is suspected, more advanced imaging or a minor procedure called laparoscopy may be suggested. Since prolonged bleeding can cause anemia and severe tiredness, it is best to get evaluated with tests rather than assuming it is just PCOS. Once the cause is clear, treatment can be planned in a way that reduces both pain and blood loss.
