After a C-section delivery, it is normal to have vaginal discharge (lochia) for a few weeks, but this usually settles within 6–8 weeks. Since it has been nearly 4 months and you are still experiencing minimal bleeding, it is understandable to feel concerned.
However, because you are exclusively breastfeeding twins, your body is producing high levels of prolactin, which can delay the return of regular periods and sometimes cause irregular spotting or light bleeding.
Additionally, with a history of PCOS, hormonal imbalance can further contribute to irregular or prolonged light bleeding patterns. The reassuring part is that your scans and tests are normal, which reduces the likelihood of retained products or structural problems.
At this stage, the bleeding is most likely hormonal in nature rather than something serious. Continue breastfeeding as usual, maintain good nutrition, and monitor the pattern of bleeding.
However, if the bleeding becomes heavy (soaking pads frequently) and is associated with foul-smelling discharge, severe abdominal pain, fever, weakness, or dizziness, you should consult your gynecologist immediately. Otherwise, this can be managed conservatively, and your cycles usually regulate gradually once breastfeeding frequency reduces.
A follow-up visit with your gynecologist can help decide if short-term hormonal treatment is needed to regulate the cycle.
