I took misoprostol 6 hours ago and only have very light spotting. I expected heavier bleeding. Does this mean the tablets didn't work? How long should I wait before I'm worried something has gone wrong?

Light bleeding 6 hours after misoprostol can be within normal range, but it can also indicate the medication didn’t work as expected.

Typical timeline: Misoprostol usually starts working within 1-4 hours. Heavy cramping and bleeding, often with clots, signals the uterus is contracting and expelling the pregnancy. Most women have significant bleeding well underway by hours 4-8.

Why you might have only light bleeding: The tablets may not have absorbed well (sublingual vs. vaginal administration affects this), or dosing/timing may have been off. In some cases, the medication simply doesn’t produce a response in the first attempt.

What to do now: Wait 24 hours from when you took misoprostol.
