Risk Factors

Risk factors for severe multi-organ DIE:

• Delayed diagnosis of early endometriosis
• Family history of severe endometriosis
• Prior incomplete surgical excision
• High estrogen exposure over time
• Nulliparity and late childbearing

Who is at Risk

Women with long-standing undiagnosed endometriosis, those with severe cyclic bowel or urinary symptoms, and those with prior incomplete surgical treatment are at highest risk for multi-organ deep infiltrating disease.

Diagnosis

Comprehensive workup includes:

• Pelvic MRI with DIE protocol for full lesion mapping
• Rectal ultrasound for bowel involvement assessment
• Cystoscopy for bladder involvement
• Renal ultrasound to assess for hydronephrosis
• Diagnostic laparoscopy for complete staging
• Multidisciplinary surgical team planning

Treatment Options

Treatment requires a combined surgical approach involving gynecological, colorectal, and urological surgeons. Options include laparoscopic excision of all DIE lesions, bowel shaving or resection, bladder or ureteral surgery, and adjuvant hormonal therapy post-operatively.

The Procedure

Multi-organ DIE surgery involves:

• Pre-operative bowel preparation and ureteral stenting
• Laparoscopic approach with multidisciplinary surgical team
• Systematic adhesiolysis and excision of all endometriotic nodules
• Bowel procedures including shaving, disc excision, or segmental resection
• Ureteral dissection and reimplantation if required
• Bladder excision and repair for bladder wall involvement
• Diaphragmatic excision for thoracic endometriosis in selected cases
• Closure and intensive post-operative monitoring

After the Surgery
