Hirschsprung’s disease (HD) is a congenital condition where ganglion cells are absent from a segment of the colon, causing functional obstruction. While usually diagnosed in infancy, some cases with short-segment involvement present in adulthood with refractory constipation. Adult HD requires careful diagnosis and surgical management.

Symptoms

Symptoms of adult Hirschsprung’s disease include:

  • Lifelong severe constipation unresponsive to laxatives
  • Abdominal bloating and distension

Causes

Hirschsprung’s disease is caused by:

  • Failure of neural crest cells to migrate to the distal bowel during foetal development
  • Mutations in the RET proto-oncogene (most common genetic cause)
  • Other gene mutations: EDNRB, EDN3, SOX10, PHOX2B
  • Associated with Down syndrome in some cases
  • Multifactorial genetic inheritance in most cases

Types of Hirschsprung's Disease

Based on length of aganglionic segment:

  • Short-segment HD (most common, rectosigmoid involvement)
  • Long-segment HD (extends beyond sigmoid colon)
  • Total colonic aganglionosis
  • Ultra-short segment HD (most likely to be missed in childhood)

Risk Factors

Risk factors include:

  • Family history of Hirschsprung’s disease
  • Male sex (4:1 male to female ratio)
  • Down syndrome (trisomy 21)
  • Other congenital anomalies
  • RET gene mutations in family members

Who Is at Risk

Males with a family history of HD, those with Down syndrome, and individuals with long-standing refractory constipation diagnosed since childhood are at highest risk for adult-presenting Hirschsprung’s disease.

Diagnosis

Diagnosis involves:

  • Contrast enema (barium) showing transition zone
  • Anorectal manometry (absence of recto-anal inhibitory reflex is diagnostic)
  • Rectal suction biopsy (gold standard) showing absence of ganglion cells
  • Full-thickness rectal biopsy if suction biopsy inconclusive
  • Genetic testing for RET and other mutations

Treatment for Hirschsprung's Disease (Adult)
