Types of Hysterectomy

Total abdominal hysterectomy removes the uterus and cervix. Subtotal or partial hysterectomy removes only the uterus, leaving the cervix. Bilateral salpingo-oophorectomy involves simultaneous removal of both fallopian tubes and ovaries and is performed when indicated by the underlying condition.

Open abdominal hysterectomy is recommended when the uterus is very large due to fibroids, when extensive adhesions from endometriosis or prior surgeries make minimally invasive approaches unsafe, when cancer requires radical removal of surrounding tissues, when the patient is not a suitable candidate for laparoscopic or vaginal approaches, or when conversion from minimally invasive surgery becomes necessary.

Pre-operative evaluation for open abdominal hysterectomy includes:

  • Pelvic examination to assess uterine size, mobility, and tenderness
  • Pelvic ultrasound to identify fibroids, ovarian pathology, and uterine size
  • MRI pelvis for detailed mapping of fibroids, endometriosis, or cancer
  • Endometrial biopsy to rule out cancer in cases of abnormal bleeding
  • Cervical smear (Pap test) to assess cervical health
  • Blood tests including complete blood count to assess anemia
  • Cystoscopy or colonoscopy if adjacent organ involvement is suspected

Surgical Procedure

Open abdominal hysterectomy is performed under general or spinal anesthesia. A horizontal or vertical incision is made in the lower abdomen. The uterus is detached from its ligaments, blood vessels, and the vaginal vault, then removed. If indicated, the ovaries and tubes are also removed. The vaginal cuff is closed and the abdominal incision is sutured. Surgery takes 1 to 3 hours.

Open Hysterectomy Procedure Steps

The open abdominal hysterectomy procedure involves:

  • Administration of general or spinal anesthesia
  • Making a Pfannenstiel (bikini line) or midline vertical abdominal incision
  • Exploring the abdominal cavity and identifying the uterus and adjacent structures
  • Dividing the round ligaments, broad ligaments, and uterosacral ligaments
  • Clamping and ligating the uterine blood vessels to control bleeding
  • Detaching the bladder from the lower uterine segment and cervix
  • Dividing the vaginal vault and removing the uterus and cervix
  • Closing the vaginal cuff with absorbable sutures
  • Irrigating the pelvis and closing the abdominal incision in layers

After the Surgery

Recovery after open abdominal hysterectomy takes 6 to 8 weeks. Hospital stay is typically 3 to 5 days. Patients should avoid heavy lifting, strenuous activity, and sexual intercourse for 6 to 8 weeks. A vaginal discharge may occur for a few weeks as the vaginal cuff heals. Hormone replacement therapy may be discussed if the ovaries were also removed. Follow-up appointments monitor recovery and pathology results.

Potential complications of open abdominal hysterectomy include:

  • Injury to the bladder, ureter, or bowel during surgery
  • Excessive bleeding requiring blood transfusion
