I was treated with methotrexate injection for ectopic pregnancy. It has been 3 weeks and my blood tests are still showing HCG levels. How long does it usually take for hormones to return to normal, and what signs should make me worried about complications?

After methotrexate treatment, HCG levels usually start dropping within the first week and can take anywhere from 2 to 6 weeks, sometimes longer, to reach zero depending on the initial levels. The key is that the numbers should steadily decrease on each test. If you develop worsening abdominal pain, shoulder pain, dizziness, fainting, or heavy bleeding, you should seek urgent medical care as it may mean the ectopic has ruptured despite treatment. Regular follow-up blood tests are very important until the hormone levels become undetectable.
