So gastric bypass is a metabolic procedure which can induce weight loss. However, obesity being a genetic disease, weight recurrence is always a possibility over years and it is according to the physiology of the human body. It is generally not called a failure because patients diseases do reverse patient does have improvement and some weight recurrence happens over time.

It can happen because of the dilatation of the pouch. It can happen because intestines learn to absorb more which is called intestinal adaptation. It can also happen because effectiveness of and secretion of certain hormones can get blunted over time and these are compensatory mechanisms created by nature.

However, luckily there are treatment options available and second stage after gastric bypass is a known treatment for patients who have weight recurrence after gastric bypass. In the second stage the dilated stomach can be trimmed and made normal.

The dilated stomach and jejunum anastomosis can be trimmed and made normal. The length of bypass can be increased or procedure can be converted into a more effective procedure like duodenal switch or a transit by partition. This is a standard procedure.

However, these procedures need skills and should be performed by experienced surgeon surgeons of center of excellence and surgeons who have high volume of second stage procedure. But fortunately there is a solution for these unfortunate patients who have weight recurrence but they can still lose weight and maintain it for many more years to come.
