A hoarse voice lasting more than 3–4 weeks should always be evaluated, especially in someone with a long history of smoking. In many cases it can be due to simple irritation, acid reflux, or vocal strain, but persistent hoarseness is also one of the early warning signs we do not ignore.

The reason we take it seriously is that long-term smoking can chronically inflame the vocal cords, and occasionally lead to more serious conditions that need early detection. The key point is not to assume it is “just throat irritation” when it is not improving over time.
A quick ENT examination with a scope of the vocal cords is a simple and very informative step. Most of the time, it turns out to be a treatable cause but the important thing is confirming it early rather than waiting longer.
