A rise in hCG from 4,665 to 8,103 after methotrexate does not by itself mean the treatment has failed. In ectopic pregnancy, hCG can initially rise after methotrexate before it starts to fall. Doctors usually assess the response by comparing the Day 4 and Day 7 hCG levels; a 15% or greater decline between Day 4 and Day 7 generally suggests that methotrexate is working.

Because your wife received another methotrexate dose, her treating doctor should closely monitor her serial hCG levels and decide the next step. Please follow the doctor’s schedule for blood tests and avoid taking additional medication without medical advice. If she develops severe or worsening abdominal pain, shoulder tip pain, dizziness, fainting, or heavy bleeding, go to the emergency department immediately, as ectopic pregnancy can rupture even when hCG is falling.
