Do All Ovarian Cysts Need Treatment or Surgery?

My ultrasound report shows an ovarian cyst. Can ovarian cysts go away on their own, or do they need treatment? If the cyst becomes large, will surgery be required? Does having an ovarian cyst mean there is a risk of cancer?

Ovarian cysts are a very common finding and, in most cases, they are not dangerous. An ovarian cyst simply means a fluid-filled sac that forms in or on the ovary. During a normal menstrual cycle, small cysts of about 2–3 cm can form naturally; these are called functional cysts and usually resolve on their own without any treatment.

In such cases, doctors often advise observation and a follow-up ultrasound, and there is no need to panic. These cysts do not increase the risk of cancer.

Ovarian cysts are broadly classified as simple or complex. Simple cysts that are small are usually managed with monitoring or medication. If a cyst grows larger, typically more than 5 cm, or shows complex features on ultrasound, further evaluation is required to understand its nature.

Simple cysts up to around 7–8 cm may sometimes still be observed with close follow-up, but larger cysts or complex cysts often require surgical treatment. Before deciding on surgery, doctors may recommend additional tests to determine the type of cyst and the most appropriate treatment.

Most ovarian cysts are benign, and cancer is rare, especially in younger women. Proper evaluation, regular follow-up, and timely treatment help ensure safe management and prevent complications.
