My chest appears slightly saggy, and the doctor has diagnosed it as Grade 2 gynecomastia. If both fat and gland are removed during surgery, will it come back, and will the skin become loose or saggy afterward?

In gynecomastia, both fat and glandular tissue can be present, and effective treatment involves removing both—fat is removed through liposuction, and the gland is completely excised through a small incision. Once the gland is fully removed, it does not grow back.
Fat accumulation, however, depends on your body weight. If your weight remains stable, fat is unlikely to return, but with weight gain, fat can accumulate again in the chest area.
Regarding skin looseness, patients are usually advised to wear a compression garment for a certain period after surgery. This helps the skin contract and adhere to the chest contour, reducing the chances of sagging.
With proper post-operative care, the skin generally tightens well, and the overall chest shape appears firmer and more defined.
