How to tell if puffy nipples are gynecomastia or chest fat?

My chest is puffy around both nipples and even though i am not really overweight. I do workout and tried loosing body fat but still it looks the same, which makes me very uncomfortable in fitted T shirts. Is this gynecomastia or just chest fat, and what treatment would help me get a flatter chest?

Puffy nipples despite being lean and working out can indeed be a sign of gynecomastia, which is the enlargement of glandular breast tissue in males, rather than just excess fat.
Unlike chest fat, gynecomastia does not respond to diet or exercise since it involves actual glandular tissue. A doctor can confirm the diagnosis through a physical examination and, if needed, an ultrasound. Surgical removal of the glandular tissue (gynecomastia surgery) is typically the most effective treatment for a flatter, firmer chest contour.

Palpate the area around nipple, I mean feel with one hand and with other hand feel the area beside the navel, if chest feels more firm it means it’s a glandular tissue, means true gynecomastia.
