40 days after bilateral microscopic varicocele surgery, some ongoing internal discomfort is still within normal range. External stitches close up fast, but internal tissue heals on its own timeline, that hard or tight area you’re feeling is typically scar tissue or residual swelling working itself out, which takes weeks, not days.

Most people are fully recovered somewhere between 6 and 12 weeks, and for some it’s closer to 3 months. If the pain is slowly improving and there’s no redness, fever, increasing swelling, or discharge, you’re probably just in the middle of a normal healing process. That said, if things stop improving or start getting worse, it’s worth seeing your doctor, not to alarm you, just to rule things out.
