Bleeding one month after a TLH (Total Laparoscopic Hysterectomy) is not considered normal, but it can happen due to a few medical reasons. The most common cause is delayed healing of the vaginal cuff (the area where the uterus was removed) or a small stitch-related issue such as a granuloma.
Infection at the surgical site can also lead to bleeding, especially if there is foul-smelling discharge, pain, or fever. Sometimes bleeding may occur if the patient resumes heavy physical activity or sexual intercourse too early.
In rare cases, mild spotting can be related to hormonal changes if the ovaries were preserved. Any bleeding that is heavy, fresh red in color, associated with pain or fever, or continues for several days should be evaluated by the operating gynecologist.
A pelvic examination or ultrasound may be needed to identify the cause, as timely treatment usually resolves the problem safely.
