Hearing Loss After Head Injury : Causes, Diagnosis & Surgical Treatment Options

I have been experiencing hearing loss in my right ear for the past 4–5 years. It started after I had a head injury where I needed stitches, but at that time no CT scan was done. Over time, my hearing has gradually worsened-from around 50% loss a year ago to nearly 70% now. I consulted an ENT specialist and underwent audiometry, and recently I was advised procedures like stapedotomy or similar surgeries, but I am confused about the exact problem and the difference between these treatments. Could you please explain what might be causing this hearing loss, whether it is related to the injury, and what the best treatment or surgical option would be in my case?

Based on your history and audiometry reports, your hearing loss appears to be related to an issue in the middle ear, most likely involving the tiny hearing bones (ossicles). Since you had a head injury in the past, there is a possibility that these bones may have become disconnected, stiff, or damaged, which can gradually reduce hearing over time. The A-S type pattern in your audiometry suggests that the movement of these bones is restricted.

To accurately diagnose the exact problem, a procedure called tympanomastoid exploration is usually recommended. In this surgery, the ear is examined internally using an endoscope, and depending on the findings, the surgeon may reconnect the bones, replace them, or correct any fixation. In many cases, this can significantly improve hearing, although outcomes depend on the exact condition found during surgery.

The procedure is generally minimally invasive, often done through the ear canal, with a short hospital stay. However, proper post-operative care, including rest and avoiding pressure on the ear, is very important for successful recovery.
