A fistula can recur after treatment if the entire infected tract did not heal completely, if there are hidden branches of the fistula, or if infection continues inside the area. Conditions like diabetes, Crohn’s disease, poor wound healing, smoking, or repeated abscess formation can also increase the risk of recurrence. Sometimes the external wound heals, but the deeper tract remains active and causes the fistula to return later.
Recurring fistula symptoms such as pain, swelling, pus discharge, fever, or repeated boils should be evaluated again by a colorectal or general surgeon. Further imaging, medicines, or another procedure may sometimes be needed depending on the complexity of the fistula.
