Most important what symptoms are experiencing, for most pts with grade 3 – 4 haemorrhoids, fissure symptoms will be more pronounced as piles complaints, need to equate the symptoms and examine to identify any active fissure is there or not, along with sometimes it Could be recurrence, of any reason. More likely pt diet history and travel history matters alot.
Probably recurrence will be due to post anorectal surgery constipation, proper post operative care should be taken

Surgery honey ke baad bhi, hame hamare aadaton ko badalna padta hai, sabzi zyada lena padta, roti aur chawal kam, pani zyadah lena padta, har din 3 litre se bhi zyadah and weight pe bhi dhyaan dena padta. Aur agar aapke kaam mein lambe samay tak baithna padta hai toh usey modify kariwn ki aap har ghante mein 10 minute tak chalti rahe.
