What is a Varicocele?

What are the Types of Varicoceles?

Varicoceles can be classified into the following types:

  • Stop-type varicoceles: In this type of varicoceles, the internal spermatic (testicular) vein fills up with blood.
  • Shunt-type varicoceles: The buildup of blood damages the spermatic vein and other veins, leading to infertility.

Varicocele can be classified into three grades:

  • Grade I: In this stage, the varicocele is not visible and can only be felt when the patient is straining, which is why the physician uses a Valsalva maneuver.
  • Grade II: At this stage, the varicocele can be felt at rest. Although it is not visible, it can be detected without using a Valsalva maneuver.
  • Grade III: This is the stage where the varicocele becomes clearly visible and can lead to complications that would involve discomfort and infertility.

Treatment Procedures for Varicoceles

  • Percutaneous embolization: This is one of the least invasive day-care procedures. During this procedure, the surgeon makes a small incision in the skin, usually around the groin, through which a small catheter is inserted into the scrotal vein. Then, a coil is inserted through the catheter into the varicocele. This blocks the affected veins, redirecting the blood flow to the circulatory system via an alternate pathway through the healthy veins which results in the collapsing of the damaged veins. 
  • Microscopic varicocelectomy: This procedure is generally performed if there is a risk of infertility. In this process, the surgeon makes a tiny incision above the scrotum. With the help of a microscope, the surgeon separates the testicular arteries and vas deferens in the scrotum. Keeping the lymphatic drainage intact, the surgeon then dissects down to the spermatic cord. After the abnormal veins are encountered, each vein is meticulously dissected circumferentially and tied off to disrupt the flow, before draining the blood away from the testicle into the inner thigh and pelvis.
  • Laparoscopic varicocelectomy: This process is mostly preferred in treating patients diagnosed with a second or third-grade varicocele and is performed using general anesthesia. During the procedure, the surgeon makes small incisions in the abdomen. Through one of the incisions, the surgeon inserts a thin lighted scope (laparoscope) and inflates the abdomen using CO2 gas, which enables them to have a clear view of the internal organs. Once the swollen veins are identified, they are cut and the ends are closed off. As soon as the ends are sealed, all the tools are removed and the incisions are closed with sutures or clips and dressing is applied.

What are the possible risks associated with Varicoceles treatment?
