About Bartholin Cyst

Bartholin cyst develops when the duct of the Bartholin gland, located on either side of the vaginal opening, becomes blocked. This causes fluid to accumulate and form a painless lump that may grow over time. While many cysts remain small and harmless, they can become painful or infected if left untreated. Early evaluation helps prevent discomfort and ensures timely treatment.

Bartholin Cyst can be treated in multiple ways depending on its size, severity, and symptoms.

Small and non-infected Bartholin cyst: If you can feel the fluid-filled lump but there is minimal pain, it means, the cyst is not infected, but the Bartholin gland's fluid has backed up.
Infected and painful Bartholin cyst: A swelled and painful cyst means it has developed an abscess (pus) because of bacterial infection. In this case, antibiotics and pain-relieving medicines are suggested.
Large, infected, and extremely painful Bartholin cyst: A large, swollen cyst with unbearable pain and pus requires surgical drainage.
Painful and recurring Bartholin cyst: If the Bartholin cyst keeps forming, again and again, marsupialization procedure is suggested.

Diagnosis & Treatment

Diagnosis for Bartholin Cyst

To detect the severity and course of treatment for Bartholin cysts, you may be recommended the following tests:

Pelvic Exam Test: Here the gynecologist carefully examines your pelvic area and understands the size, nature, and severity of the cyst.
Swab Test: Here the gynecologist collects a sample of vaginal fluid on a swab to test it for infections.
Biopsy: If the woman is post-menopausal, there is a chance of carcinoma cells. Hence, the gynecologist may take a sample cell and perform a biopsy to screen cancer.

Marsupialization for Bartholin Cyst

Marsupialization: It is a minor surgical procedure performed under local/ spinal/ general anesthesia depending on the severity of the cyst/ patient’s personal choice.

The gynecologist first makes a nick to drain the pus and then enlarges the incision slightly to create a temporary ‘kangaroo pouch’ on the labial lips. This helps support fuller drainage over a period of a few weeks. If needed, the ends of the incision may be stitched outwards or a catheter may be placed to drain the pus.

This method helps push the complete pus out of the cyst which may or may not be otherwise visible to the naked eye and thus helps prevent Bartholin cyst recurrence.

How to prepare for Marsupialization surgery?

Here is a general guideline to help you prepare for the surgery:

Discuss all your current medicines: Discuss your health conditions, medicines, supplements, and herbal potions elaborately with your doctor. Protect the area from any injury: Be careful as you work/ play or do any physical activities. Injury on the Bartholin gland can cause trauma and back up Bartholin gland’s fluid, eventually forming a Bartholin cyst.
Practice safe sexual habits:: Use a condom and maintain the highest levels of sexual hygiene. Basically, though Bartholin cysts can occur because of various reasons, STI is one of the major reasons it gets infected and turns into an abscess. Therefore, using a condom can help prevent STI and pus formation.
Careful Penetration:: Rough/ dry sex or negligence while penetration can tear the skin around labia and clog Bartholin glands, causing painful Bartholin cysts. Therefore, being careful and using lubes is advised.

Can I eat anything before Marsupialization surgery for Bartholin cyst?

In some cases, yes. If the procedure is performed under local anesthesia, fasting is not required. You can eat as you like. However, if the same is performed under spinal anesthesia, fasting is suggested for at least 4-6 hours before the surgery. Which one is better for you, is best decided by your operating doctor. Therefore, please confirm the same for the specifics.

How long does the Bartholin cyst surgery take?

Bartholin cyst removal surgery is usually an under 60 minutes procedure. However, this time may also vary basis your individual health, co-morbidities, and doctor’s experience.

Does Bartholin cyst surgery (marsupialization) require a hospital stay?

No. Not always. In most cases, marsupialization is a daycare procedure, that is, you can go home the same day post-treatment. However, in some cases, 1-day hospitalization may be suggested. These specifics vary on your individual health and condition’s severity.

Gynecologists suggest you wait for at least 10-15 days before engaging in penetrative sex. This is to ensure complete recovery and avoid any infection. However, this may differ from case to case. Please confirm the same directly from your operating doctor.

Does marsupialization of Bartholin cyst affect a woman’s fertility?

No. Marsupialization has no negative effect on a woman’s fertility. The procedure is performed only on the external vaginal walls while the reproductive organs such as ovaries, fallopian tubes, or the uterus, lie much above it.
