What Is Sinusitis?

Sinusitis is a condition in which the inflammation of the sinuses occurs. Sinuses are air-filled spaces positioned behind the forehead, cheeks, and eyes. This condition can arise due to various factors, including viral or bacterial infections, allergies, or structural problems in the nasal passages. Common symptoms of sinusitis include facial pain or pressure, nasal congestion, headaches, thick nasal discharge, and a diminished sense of smell. The treatment of sinusitis depends on the severity of the condition and symptoms you are experiencing.

Stages of Sinus Infection

Stages of sinusitis on the basis of the severity of the condition

There are three types of sinusitis on the basis of the severity of the condition:

  • Acute sinusitis: In this type of sinusitis, symptoms are present for 4 weeks or less. It is caused by bacteria growing in the sinuses.
  • Chronic sinusitis: In chronic sinusitis, symptoms and swelling of the sinuses last for longer than 3 months. It may be caused by bacteria or a fungus.
  • Subacute sinusitis: In subacute sinusitis, symptoms and swelling usually last up to 1 to 3 months.

Stages of sinusitis on the basis of their location

There are 4 types of sinusitis on the basis of their location:

  • Maxillary sinus – Either side of the nose within the cheekbones.
  • Ethmoidal sinus –Either side of our nose in between the eyes.
  • Frontal sinuses – Within the forehead bone.
  • Sphenoid sinus – Situated behind the nose.

Sinusitis Diagnosis

A doctor usually begins diagnosis by evaluating medical history and a thorough physical examination to check for tenderness in the nose and throat. To examine the nose more precisely, a healthcare provider uses a medical device to open the nose and apply medicines that constrict the blood vessels in the nose. A healthcare provider may flashlight inside the nose to check for inflammations, fluids, nasal polyps, and tumors. Moreover, a doctor usually recommends the following diagnostic tests to understand the severity of the condition:

  • Nasal endoscopy: In this test, a tube is inserted into the nose using the aid of a fiber-optic light. It helps the healthcare provider to check the inside of the nose very easily.
  • Imaging studies: A doctor usually recommends CT and MRI scans to check for inflammations of the sinuses.
  • Nasal and sinus cultures: If the sinusitis is caused by microbes such as bacteria or fungus infections, then cultures may be performed to diagnose the cause of the disease correctly.
  • Allergy test: If a healthcare provider suspects that sinusitis is caused by allergies, then he/she is likely to suggest an allergy test to confirm the condition.

Sinusitis Infection Treatment Options

The treatment of sinusitis depends on the severity of the condition and the symptoms you are experiencing. There are two treatment options for sinusitis, surgical and non-surgical.

Sinusitis Surgical Treatment

A healthcare provider usually recommends surgerywhen the symptoms of sinus infection become severe and unmanageable through medicines and therapies. In most cases of chronic sinusitis, a doctor recommends surgery. A sinus surgery generally involves removing the infected sinus, nasal polyps, or bone or treating other conditions that might be causing the symptoms. The three most commonly performed sinusitis surgeries and their procedures are as follows:

  • Functional endoscopic sinus surgery (FESS):

FESS is the most commonly performed surgery for the sinus. It is usually performed to widen the passages between the nose and sinuses to remove bone-infected tissues to allow the trapped mucus to drain out. This surgery is performed with the help of an endoscope, which allows doctors to look inside the nose and sinuses and perform the surgery with precision. During the surgery, a doctor puts a decongestant and numbing solution in the nose. After that, a surgeon inserts surgical tools along with the endoscope to extract bone, damaged tissue, or polyps that may be causing the blockage in your sinuses. Finally, the doctor packs the nose with bandages to soak blood or discharge.

  • Balloon sinuplasty:

Balloon sinuplasty is a minimally-invasive surgery usually performed to treat sinusitis with the help of an endoscope. A small balloon is inserted into the nose with the help of an endoscope and catheter, which increases the passageway to the sinus. The surgery is performed under the influence of general anesthesia. During the procedure, a doctor gives anesthesia to sedate the patient. It is injected in the tissue lining of the nose. After that, the doctor inserts a catheter into the nose with the help of an endoscope. The doctor then puts a small balloon in the sinus and inflates it slowly to unblock the sinuses, and lastly, the balloon is removed.

  • Caldwell Luc surgery:

Caldwell Luc Surgery is performed when other treatment options fail to relieve the condition. In this surgery, doctors access the sinuses through a new opening in the maxillary sinus, which is located behind the neck. The surgery is performed under the influence of general anesthesia to sedate the patient. After that, a healthcare provider makes an incision in the gum, between the upper lip and gum tissue, to access the wall of the maxillary sinus. In the next step, a small hole is made in the sinus wall to remove the damaged tissue or bone which is causing the problem. An endoscope is used to widen the opening of the sinus. Finally, sutures are used to close the gum incision.

Benefits of Sinusitis Surgery

Sinusitis surgery provides numerous benefits for individuals facing severe or recurrent sinusitis symptoms. Here are some advantages of sinusitis surgery:

  • Enhanced sinus drainage: The primary goal of surgery is to remove blockages or obstructions in the sinuses to improve mucus drainage. As a result, the frequency and severity of sinusitis symptoms decrease.
  • Relief from chronic sinusitis symptoms: Consistent sinusitis symptoms such as facial pain, pressure, congestion, headaches, and post-nasal drip can be relieved by sinus surgery. It significantly improves overall well-being and the quality of life.
  • Improved response to medical treatments: Sinusitis surgery can improve the efficacy of drugs and other therapies for sinusitis by removing physical obstacles and enhancing sinus drainage. This enables more effective medicine delivery and absorption to the afflicted areas.
  • Correction of structural issues: A deviated septum, nasal polyps, or a nasal bone are examples of structural abnormalities in the sinuses or nasal passageways that are treated by sinus surgery. The procedure can help avoid or lessen the onset of chronic or recurrent sinusitis by addressing these problems.

How Can I Prepare Myself For Sinusitis Surgery?

  • A healthcare provider usually suggestsmedications to reduce inflammation and infection of the sinuses before the surgery. Medications to reduce respiratory secretions are usually given the night before the surgery.
  • Patients who routinely take aspirin, anticoagulants, or NSAIDs should cease taking them at least two weeks before surgery since they increase the risk of bleeding.
  • You must avoid smoking for at least a month prior to surgery and for at least a month afterward since it prevents quick recovery.
  • The night before the procedure, you will need to check into the hospital. You will not be allowed to eat or drink after midnight. You can occasionally need intravenous liquid to stay hydrated.

Recovery After Sinusitis Surgical Treatment

You must follow all the instructions provided by your healthcare provider to promote healing after the surgery. It may include wound care, medication usage, and nasal care.

  • Initially, there may be discomfort, pain, and swelling, but these symptoms can be managed with prescribed pain medications. Proper rest is crucial during this time to promote healing.
  • You must avoid excessive exercise during the recovery period as it can delay tissue healing.
  • Nasal saline rinses or sprays can treat common nasal symptoms like congestion, discharge, or slight bleeding.
  • You must attend all the scheduled follow-up appointments with your doctor to track your recovery and resolve any issues. The recovery period after surgery can vary depending on the specific condition.
  • For a successful and hassle-free recovery, it’s essential to follow all the instructions provided by your healthcare provider.

Risks and Complications of Sinusitis Surgery

Like any surgical procedure, sinusitis surgery carries certain risks and complications, including:

  • Bleeding: There is a risk of bleeding during and after the surgery. If you experience heavy bleeding for more than one week, then you must consult your healthcare provider to avoid the chances of complications.
  • Infection: Any surgical procedure poses a risk of infection. There is a rare possibility of developing an infection at the surgical site or within the sinuses after the surgery. A doctor usually prescribes antibiotics to minimize the risk of infection.
  • Scarring: There is a chance of scarring as the surgery involves incisions and tissue removal. In some cases, excessive scarring may cause narrowing of the nasal passages and affect normal airflow.
  • Changes in smell or taste: There is a chance that temporary or permanent changes in the sense of smell or taste can occur following sinus surgery. This can be due to disruption of the olfactory nerves or swelling of the nasal tissues.
  • Facial pain or pressure: It is common to experience facial pain or pressure immediately after surgery. This discomfort typically subsides with time, but in rare instances, it may persist.
  • Eye or vision problems: In some rare cases, sinusitis surgery can potentially lead to eye-related complications such as double vision, vision loss, or injury to the eye. Prompt medical attention is required in such cases.
  • Cerebrospinal fluid leak: In rare situations, the surgery may cause a leak of cerebrospinal fluid (CSF) surrounding the brain. Symptoms may include clear drainage from the nose, headaches, or an increased risk of infection.

Lifestyle Changes After Sinusitis Surgery

A doctor usually recommends certain lifestyle changes after undergoing sinusitis surgery to promote healing and reduce the risk of complications. These changes can help improve the outcome of the surgery and support long-term sinus health. Here are some common lifestyle adjustments that may be suggested:

  • To keep the nasal passages moist, remove mucus, and avoid crusting following surgery. Examples include gentle saline rinses or prescribed nasal sprays.
  • Avoiding irritants that aggravate sinus symptoms or slow down healing. You must avoid allergens, smoking, potent odors, and environmental contaminants that can irritate and inflame the nasal passages.
  • A healthcare provider usually advises using a saline solution for sinus irrigation to clear mucus and promote healing. To ensure safe and efficient sinus irrigation, it’s crucial to employ the right method and sterile tools.
  • It is crucial that you adhere to the detailed post-operative instructions that your healthcare provider gives you.
  • It’s critical to handle allergy and sinusitis causes effectively. This may entail taking allergy medications as directed, staying away from recognized triggers, and keeping an allergen-free environment.

What Happens in Sinus Surgery?

Sinus Surgery is a minimally invasive procedure which is called FESS used to treat chronic sinusitis and other sinus issues. During FESS, an ENT surgeon inserts an endoscope — a thin, flexible tube with a camera—into the nostrils to visualize the sinus passages. Using specialized instruments, the surgeon removes obstructions like polyps, diseased tissue, or bone, improving sinus drainage and airflow. This approach reduces tissue damage, minimizes bleeding, and promotes quicker recovery compared to traditional sinus surgery. The procedure is typically performed under general or local anesthesia and often results in significant relief of sinus symptoms.
