This is the most severe stage in which chronic toenail deformity is present in both lateral and distal nail folds. The hypertrophic tissue entirely covers the lateral, medial, and distal nail plate. In young patients, wedge resection is done using the Winograd technique. And in adults, total matricectomy with phenol is recommended.
