What is Prostate Enlargement?

Benign Prostatic Hyperplasia, or BPH, is a condition observed in men over 50 years of age. In this condition, the prostate gland is enlarged causing a hindrance in the flow of urine. The condition is benign, which means that it is not cancerous. The term ‘prostatic’ is associated with the prostate gland in males which is located below the urinary bladder and surrounds the urethra. ‘Hyperplasia’ means the enlargement of an organ. People with BPH have their prostate enlarged as a result of which the urethra becomes narrow and eventually the bladder weakens and loses the ability to empty urine completely from the bladder. The main function of the prostate gland is to secrete a fluid that is a part of the semen.

Treatment

Diagnosis

When the symptoms begin to show, or if there is some family history of Benign Prostatic Hyperplasia, then it is better to consult a doctor and get some physical exam and medical tests done. The tests which may help to diagnose BPH are:

  • Urine test
  • Digital rectal exam
  • Blood test
  • Urinary flow test
  • Prostate-specific antigen (PSA)
  • Post-void residual volume (PVR)
  • Uroflowmetry

Procedure

Non Surgical Treatments:

  • Medicines: Alpha Blockers are pills used to relax the muscles of the prostate and bladder. They improve urine flow and reduce the blockage caused by the urethra. 5-Alpha Reductase Inhibitors block the production of DHT. DHT is a hormone that plays a vital role in the growth and development of the prostate. The pills in this method increase urine and reduce the size of the prostate as well.
  • Combination Therapy is the use of alpha blockers and 5-alpha reductase inhibitors together. It makes the treatment more effective as both the classes of medication make the symptoms go away easier.
  • Phytotherapies are herbal therapy where the popular herb palmetto is used. However, it is not recommended by healthcare providers.

Surgical Treatments:

Minimal surgeries or procedures can be done to keep the situation under control depending on the level of seriousness of the patient. They include:

Transurethral resection of the prostate (TURP)

In this, a combined visual and surgical instrument (i.e., known as resectoscope) is typically inserted from the tip of the penis of an individual and the tube is passed into the urethral tube which carries urine from the bladder. The prostate usually surrounds the urethra. With the help of resectoscope, the doctor may sometimes trim away the excess of prostate tissue which is blocking the urine flow. TURP is generally considered as an option for the men who are suffering from moderate to severe urinary problems who haven’t found relief or responded to the medication.

Photo selective vaporization

Laser PVP surgery utilizes a cystoscope inserted into the penis. The excessive tissue of the prostate is burned or vaporized in order to help the individual pass the urine easily

Holmium laser prostate surgery (HOLEP)

is a minimally invasive treatment that uses a laser to remove tissue from an enlarged prostate. A specialized instrument is used to cut the prostate tissue into small fragments that are easily removable.

Can BHP lead to Cancer?
