What is a chocolate cyst?

Chocolate cysts are fluid-filled cysts that usually form deep within ovaries and are non-cancerous. In medical terms, chocolate cysts are known as Ovarian Endometrioma. They are commonly found in women of reproductive age. The symptoms of a suspected case of the chocolate cyst are non-specific. Most women are not able to distinguish between the abdominal pains associated with menstrual cramps and those caused by chocolate cysts. Chocolate cysts most commonly grow on the ovaries because they have the largest amount of estrogen hormone that provides ideal conditions for growth. During menstruation the thin coating of the endometrium peels off and combines with menstrual blood that is discharged from the vagina. Some of the blood that does not escape, but flows back down the fallopian tubes gets collected in the endometrium. The endometrial cells are like seeds that slowly get bigger and grow into cysts. According to the World Health Organization, chocolate cysts affect roughly 10% of reproductive-age women and girls globally.

Diagnosis and treatment of chocolate cyst

Diagnosis of chocolate cyst

Doctors start the diagnostic procedure with a physical examination in which they examine the swelling and redness caused by the cyst. If a doctor suspects the case of chocolate cysts, then he/she might recommend a few diagnostic tests to confirm it. Some common diagnostic tests that doctors recommend are as follows:

  • Pelvic ultrasound: The doctor recommends this test to identify whether the cyst is present or not. But this test can’t necessarily determine the type of cyst.
  • Needle biopsy: To perform a needle biopsy, the doctor inserts a needle through the vagina into the chocolate cyst. After that, the doctor extracts the fluid and debris from inside the cyst. The doctor examines the extracted fluid under a microscope. This test helps in identifying the type of cyst.
  • Blood test: This test is usually performed to check any abnormalities linked to the hormones. It also helps in determining the levels of protein markers associated with ovarian cancer.

Treatment of a chocolate cyst

The treatment of a chocolate cyst depends on several factors, including the  age of the woman, symptoms she’s experiencing, and her future plans for pregnancy. The treatment of chocolate cysts is broadly categorized into medical management and surgical management.

Medication: In medication, the doctor prescribes birth control pills to stop ovulation, which helps prevent the development of new cysts. In some cases, the doctor recommends non-steroidal anti-inflammatory drugs (NSAIDs) to manage chocolate cyst pain. Medication helps in temporarily managing the pain, but it can’t necessarily cure them.

Surgery: Generally, two surgical techniques are used to remove chocolate cysts – laparotomy and laparoscopy.

Laparoscopic surgery: Laparoscopic surgery is performed under the influence of spinal or general anesthesia. This surgery is performed to remove chocolate cysts with the help of a laparoscope. During the surgery, the doctor makes an incision through which they insert a laparoscope. A laparoscope is a long thin, tube-like medical device with a light and camera on its end.

Laparotomy surgery: It is the traditional, open surgery for chocolate cyst that involves a large incision. During laparotomy, the patient is sedated with general or spinal anesthesia. Then, the surgeon makes a large incision in the stomach to get a better access to the cyst. After that surgeon removes the cysts and uses stitches or staples to close the incision. The surgeon sends the cyst sample to the laboratory to check whether it’s cancerous.

Frequently Asked Questions around chocolate cysts

How to reduce the size of chocolate cysts?

Doctors usually recommend medication that inhibits ovulation, such as birth control pills for small-size asymptomatic chocolate cysts. This can help control pain and slow down the growth of cysts, but it can’t cure chocolate cysts. If women experience painful symptoms, the doctor is likely to recommend surgery to remove the cysts.

What can I drink to shrink cysts?

Herbal teas like chamomile tea and green tea are considered a good remedy to shrink the cysts. Mix two teaspoons of dried chamomile and one teaspoon of honey in a cup of hot water. Cover and leave it for five minutes. You can drink two to three cups of chamomile tea daily until you get rid of the problem.

How to stop chocolate cysts from growing?

The doctor recommends a few medications to inhibit ovulation, such as the birth control pill. This can help control the pain and slow down the growth of cysts, but it can’t cure them. But if you have recurrent cysts that start causing painful symptoms, then a doctor is likely to suggest surgery to remove cysts.

What are the risk factors of chocolate cysts?

Some certain aspects that make women more prone to developing chocolate cysts in the ovaries are as follows:

  • Prominent hormonal problems like thyroid, and PCOS
  • Regular intake of fertility drugs, that cause harmful fluctuations in progesterone, and estrogen levels
  • Infection in pelvic region
  • Prior occurrences of ovarian cysts

Which food items are good for the treatment of chocolate cyst?

Foods that are rich in fiber such as pears, oranges, lentils contain phytochemicals that stop the re-absorption of estrogen hormone in the body. They further reduce the risk of ovarian cancer.

Can chocolate cysts go away naturally?

In most cases, chocolate cysts go away on their own and don’t require medical attention. If you are experiencing unpleasant symptoms, your healthcare provider may recommend treatment based on the severity.

Is it necessary to remove the chocolate cyst?

If the chocolate cysts start causing unpleasant symptoms or put you at risk, you may require specific treatment. Doctors first diagnose the condition to understand the severity of the condition and plan the treatment accordingly.
