What is abortion? 

Abortion, as the name suggests, is the medical procedure to abort (end) an unwanted pregnancy. It is legal for both married and unmarried women in India and permissible until 20 weeks under the following four conditions:

  1. Danger to the physical and mental health of the mother or the baby
  2. Chromosomal abnormalities in the fetus
  3. Failure of Contraception
  4. When the pregnancy is the result of sexual assault, such as rape/ incest.* (permissible until 24 weeks on approval by 2 doctors)

Abortion is an extremely personal decision, and if you are above 18, you do not need your partner/ parent's approval for it. You can request to undergo the procedure with your own written consent. Types of Abortion

There are primarily two types of abortion, medical and surgical. Both these types of abortion are safe and a woman can choose one of them depending on the convenience or how far she is in terms of her pregnancy. There are many women who prefer having medical abortion whereas others prefer having a surgical abortion.

Medical Abortion

Medical Abortion is done using medicines or pills. This method is only available for a woman up to 10 weeks of pregnancy. It involves using two different medicines, i.e., mifepristone and misoprostol. The first pill is given by the doctor in the clinic whereas the second one is taken at home by the patient herself. Mifepristone blocks the necessary hormone for pregnancy and detaches the embryo from the uterine wall. Misoprostol will cause the uterus to contract and the cervix will open slightly to expel the pregnancy. The expulsion will be a painful time as you will experience heavy bleeding with cramps.

Surgical Abortion

Surgical Abortion is done on an outpatient basis and involves the removal of the embryo using specialized instruments like a suction cannula. It can be done at any time until the 24th week. Depending on the time that has passed, the doctor may utilize other methods like dilation and evacuation to remove the embryo.

The surgical procedure of abortion is carried out under the influence of general anesthesia. The patient is then asked to lie in a lithotomy position. In lithotomy position, the patient is required to lie down on their back with the legs flexed 90 degrees to the hips. This position allows the surgeon to extract the fetus from the uterus through the birth canal.

The vaginal area is then cleaned with the antiseptic solution i.e Betadine. The gynecologist then inserts a speculum into the vagina to stretch it wide. The doctor then passes a hollow tube i.e. cannula into the uterus. The cannula is attached by a tube to a bottle and a pump that provides a gentle vacuum to remove the fetus from the uterus. A curved instrument called a curette is then used to gently pull out the lining of the uterus and remove tissues from the uterus. The uterine tissues are removed during the D&E (Dilation and Evacuation) procedure. After D&E, the surgeon thoroughly examines the uterus to ensure that all the tissues are removed completely and safely. The examination is necessary to prevent the chances of the remnant fetus getting stuck in the uterus and creating complications.

When is abortion considered unsafe?

Abortion is considered to be unsafe when the procedure is carried out by a person who is not certified to perform the abortion or when the person lacks the necessary skills or the minimal medical standards that are required to perform the abortion.   • Dilation and Evacuation

Dilation and evacuation (D&E) is a surgical procedure for medical termination of pregnancy in the second trimester, i.e., after the 14th week of pregnancy. This method of abortion is chosen when a woman has to terminate the pregnancy due to severe abnormality in the fetus or some medical problem.

The process is done under anesthesia. Once the woman is sedated, the gynecologist will part her legs and examine her uterus. The doctor will insert a speculum inside the vagina. With the help of dilators and hydrogel rods, the doctor opens the cervix and softens it. Using narrow forceps, the doctor removes the pregnancy through the neck of the womb. The doctor will then insert a tube through the cervix and a suction machine will be used to complete the evacuation. The treatment will not take more than 20 minutes. After the abortion is complete, the patient will be shifted to a private ward and shall be kept under observation until anesthesia effects wear off.

  • Dilation and curettage

Dilation and curettage is a surgical procedure in which the cervix is dilated and a curette is used to remove the inner lining of the uterus.
