Teeth Aligners - Diagnosis and Procedure


Before giving the set of dental aligners to the patients, some important diagnostic exams are done by the dentist to determine the extent of correction. The following exams will be done-

  • 3D Scan- This scan is done to take impressions of the teeth so that the orthodontist can clearly see the tooth structure and check how your teeth align together. This test will help to design a new and perfect smile.
  • Dental Exam(General)– This exam is performed to ensure you don’t have dental issues, such as cavities that can interfere with the treatment. Though cavities won’t necessarily prohibit the treatment, but being aware of the condition will allow the orthodontist to customize the aligners properly.
  • Bite Registration- A bite registration helps in assessment of the relationship between upper and lower teeth. The patient is asked to bite on a piece of impression material which helps assess the occlusal surfaces and relationship of the teeth.

A polyvinyl siloxane impression is used to create the mold of the teeth or an intra-oral digital scanner is used for digital scanning of the teeth.

Manufacturing of Invisible Aligners 

A 3D scan is conducted to obtain a digital model of the oral tissues which is connected to a 3D printer. This model is sent to the orthodontist for approval. After approval, the aligners are manufactured using a CAD-CAM (Computer-Aided Design & Computer-Aided Manufacturing) software and stereolithography (a rapid prototyping technique).

The aligners molds are built in layers using liquid resin that turns into a hard plastic when exposed to the laser. The elastic thermoplastic material of the aligners applies ample pressure on the teeth to move them in the correct direction.

Note– If a tooth needs to be rotated or pulled down, the tooth will be colored and a composite attachment is bonded to that specific tooth.

As the clear aligners are not as rigid as metal braces, the flexibility of the aligners is compensated for required teeth movement. Reproximation or interproximal reduction is also used sometimes to remove the enamel between the teeth to get a proper fit of the aligners. It also involves reshaping the neighboring teeth’ contact area.

How Do Aligners Work?

6 to 48 aligners may be needed to achieve the desired results. You will receive a package of aligner trays for a 6 month duration of your treatment. Each tray of aligners is customized to fit the teeth. They gradually shift the teeth into a straighter position by applying pressure. The gradual movement supports the tissues while adjusting the position of teeth. The aligner trays are to be switched every 15 days.

Each clear aligner should be worn for at least 20-22 hours a day for 1-2 weeks. At the end of the treatment when final results are achieved, the orthodontist will give you an invisible retainer (removable or fixed). The retainer will ensure that the teeth don’t move back into the previous position.
