Dental aligners, also known as clear or invisible braces, are a set of special customized dental trays that are used to treat dental misalignment. As they are clear in color and are only attached to the teeth, they are more comfortable and aesthetic in nature as compared to other braces. Aligners are also much better at providing more precise dental movement and help in achieving an almost perfect smile.

Aligners are removable sets of trays and thus rely heavily on patient compliance for their success. Moreover, if the patient gets dental caries during the treatment or is not compliant with the required aligner time, then it can lead to disruption of the entire treatment and may even require the fabrication of new trays. Most patients who come to the clinic for the dental clinic for aligner treatment are already aware that they have some degree of dental misalignment. The treatment usually begins at the consultation stage, where the surgeon assesses the condition of teeth and gums. If the patient has any dental decay or gum disease, then the dentist corrects it before beginning the treatment. 

Then, he will take oral photographs and radiographic scans like lateral cephalograms, X-rays, oral pantomogram (OPG), etc. Finally, they will perform a ClinCheck scan and start fabricating aligners using composite resin materials. Normally, the number of aligner tray sets required for correction can be from 6-48, and each tray may need to be worn for 1-2 weeks, 20-22 hours a day. 

The orthodontist will create button attachments on the buccal surfaces of 1st molars to firmly fix the aligner tray and make sure it does not slip while chewing or speaking. The treatment can last from 8 to 8 months, depending on the amount of correction needed.

For dental aligner treatment, you need to prepare for long-term oral hygiene maintenance. Your first aligner tray may feel very uncomfortable, and it may be difficult for you to figure out the best way to take out the aligners for brushing and tray cleaning. Do not use your nails as they may wound the already sensitive gums and increase the chances of gum bleeding. 

As you get used to the aligner trays, they will become second nature to you and you may even stop noticing their presence. When the treatment is completed, you will be given a choice of retainers, i.e., removable or fixed retainers to maintain the treatment results over a long period of time.

  • Aligners can also be more comfortable than braces, especially as the treatment goes on. Aligner trays are also much less likely to break compared to metal wires.
  • Though the patient may have to avoid certain foods to avoid breaking the aligner trays, there are fewer diet restrictions with aligners as compared to patients with fixed braces.
  • Due to the sharp edges of metal wires and brackets, they often injure the oral cavity during the first couple of weeks of the treatment, however, since there are no sharp edges to the aligners, they do not injure the oral cavity.

Since the tooth movement is slow and happens gradually over a long period of time, there is almost no adjustment period required. You will have to get a set of retainers, either fixed or removable, to preserve the results of the surgery. Fixed retainers are bonded to the lingual surfaces of the anterior teeth, while removable retainers are wire-resin appliances that have to be worn for a large part of the day for at least a year.

Even after you get the retainers, you should visit your dentist at least once every 6-8 months for follow-up checkups and make sure that your retainers are not bent or broken.

Are dental aligners effective?

Yes, dental aligners are very effective in treating dental malalignment. In fact, since aligner trays are pre-formed and provide a larger opportunity for manipulation, they are considered much better than fixed braces at providing more accurate results with perfect alignment.

Do aligners cover the gums as well?

No, dental aligners do not cover the gums or any part of the oral cavity other than teeth. To keep the aligners fixed and prevent them from slipping while chewing, talking, etc., the dentist creates a button-like attachment on the first molars.

Are aligners better than metal braces?

There is no set answer for this question as the effectiveness of aligners and braces depends on a variety of factors such as the degree of misalignment, the patient’s age, etc. The choice between metal braces and aligners often also depends on the patient’s budget.

Will I get all my aligner sets at the same time?

No, your orthodontist will evaluate your dental condition, teeth movement, etc. at weekly or biweekly appointments and provide you with a set of aligner trays to wear till your next appointment.

What are the risk factors that can affect the success rates of aligner treatment?

The biggest risk factor behind aligner treatment failure is its dependence on patient compliance. Most patients tend to feel a lot of discomfort when they get their first aligner tray which can discourage them from wearing it as much as they should. If the trays are worn appropriately, it can lead to further misalignment. In some cases, the remaining trays can become useless and the patient may need to begin the entire treatment again.
