Dental Braces- Diagnosis and Procedure

5 stages of orthodontic treatment (braces):

  • Consultation stage: Often, dental misalignment and malocclusion are self-diagnosed, i.e., it is easily visible to us and people around us. Most people who visit an orthodontist are either already thinking about getting braces or have already made up their minds and are looking for a good orthodontist to start their treatment.
  • Banding/bonding stage: This usually happens during the appointment following the consultation. During this stage, the braces are placed in their desired positions. The procedure is entirely painless, and it is common for patients to fall asleep during the process. During the first consultation of this stage, the dental surgeon will place the brackets and spacers. Spacers usually stay for 1-2 weeks and help create gaps in the teeth, depending on your tooth condition. After this, the spacers are taken out and bands and wires are fixed. If needed, the elastics are also placed during this time.
  • Regular adjustments stage: There is no set duration for this stage. Normally orthodontic treatments last from 12-18 months, but in very rare cases, they can last 8 months to 24 months. During this stage, you will typically attend appointments every 4-7 weeks for tightening and adjustments required. If the wire or bands are broken, they will be replaced during this time. The dentist will also constantly evaluate your dental hygiene to ensure that you do not end up with cavities or other dental problems after the surgery.
  • Debanding stage: This usually happens at the end of the treatment. The brackets are taken off and the bonding agent is removed from the tooth surfaces. The dentist will clean and polish the teeth and take the final photographs of the patient’s teeth and face for a before and after comparison. The orthodontist will also take final impressions of the patient’s teeth to fabricate retainers.
  • Retainer stage: Retainers help retain the changes in teeth alignment that are performed through the orthodontic treatment. There are two types of retainers- fixed or removable retainers. Fixed retainers are bonded to the lingual surfaces of the teeth while removable retainers can be worn by the patient at home or while they are sleeping, etc. In both cases, you should visit your orthodontist every 6-8 months to ensure the retainer is working properly.

Are braces painful?

The process of getting the braces is not painful. However, you can get mild pain, soreness, and discomfort after the brackets are put on. Are lingual braces better than aligners?

There are special toothbrushes and flossing tools that you can use to maintain dental hygiene while using braces. These brushes help brush at the gumline below the brackets. You should brush your teeth after every meal or snack to avoid food lodgement. You should also use a special antimicrobial mouth rinse and a tongue scraper to clean your mouth completely.

If my bracket falls off while eating, what should I do?

Brackets that have come off the tooth surface can be easily placed back into the mouth by your orthodontist. Make sure you store the bracket in a safe and clean container and bring it to your orthodontist immediately, if possible, or during your next appointment.

Will my face change shape after braces treatment?

Yes. Braces improve the relationship between upper and lower arches and make our facial shape more symmetrical. Depending on how bad your teeth alignment and occlusion were before the treatment, the change in your face shape may be barely noticeable or there may be considerable changes. Braces are most beneficial for young developing kids as they can help guide their jaw development in the proper position.

What kind of diet restrictions will I have to follow if I have braces?

If you are getting braces you need to avoid chewy, sticky, or hard foods such as popcorn, jellybeans, chewy toffees, nuts, chewing gums, hard candies, caramel, pizza, etc., as they can get between brackets and be difficult to remove completely.

Can I kiss someone with braces?

Close-mouth kissing is ok for the first couple of weeks after you get braces but you should be slow and careful with serious kissing. Keep your tongues away from the braces, and make sure there are no sharp edges that can injure either you or your partner.

What are the different types of braces?

There are primarily five types of braces:

  • Metal braces: Metal, or traditional, braces are the most common and affordable types of braces used all over the world. They are small, effective, comfortable, and often faster in delivering results than their counterparts like ceramic braces and aligners. They are fixed to the teeth for the duration of the treatment.

The only drawback of the metal braces is that they are aesthetically displeasing and their sharp edges can result in injury to the tissues surrounding them.

  • Ceramic braces: Ceramic braces are made of tooth-colored material so they are more aesthetic than metal braces. They are less noticeable and more affordable than other aesthetically pleasing options such as aligners and lingual braces.

The biggest drawback of ceramic braces is that they are bulkier than metallic braces and take up more space in the mouth. They can also stain if the patient does not follow proper dental hygiene.

  • Lingual braces: Lingual braces are placed on the inner surfaces of the teeth, as such they offer faster results with minimal visibility, even for complex orthodontic cases.

Lingual branches can, sometimes, cause speech impediments and tongue injury when they are first applied. These problems go away in a couple of weeks as the patients get used to their braces.

  • Clear aligners: Aligners are transparent 3-d printed sets of tooth moulds that slowly change the shape and alignment of teeth. They are removable so it is easier to maintain oral hygiene. Since tooth movement is more controlled with aligners they provide better results than braces.

Aligners are best for simple tooth adjustments and may not be as effective for complex dental movements. They also rely on patient compliance to work, so the patient’s willpower should be strong if they want this treatment.

What does a patient’s journey look like if they are getting orthodontic treatment?

Consultation stage:

The initial consultation usually lasts for about 20-40 minutes, depending on your dental condition. During the appointment, your surgeon will check your dental condition, treat any cavities you have, and explain your options to you. Once you’ve made up your mind about the type of braces you want, your dentist will make impressions of your upper and lower teeth and take digital photographs and X-rays of your teeth.

Banding/bonding stage

Your dentist will clean and polish your teeth before placing the brackets. Once that is done, they will retract your cheeks using a cheek retractor. They will dry your tooth surfaces and place a bonding agent over them and fix the brackets to their positions using a light-bonding cement. Your orthodontist will then place dental spacers around your teeth. After a week or two, the spacers will be taken out, and wires, elastics, etc., will be placed inside the brackets.

Immediately after the brackets are placed, you may have trouble adjusting to their presence in your mouth. Often the sharp edges of the brackets/spacers can snag on your cheek or lip and result in small wounds. Your dentist will provide you with dental wax to place on these sharp edges to avoid any wounds.

Regular adjustments stage:

This stage constitutes the majority of the duration of the treatment journey. You will have to visit your orthodontist every 4-8 weeks for adjustments of your orthodontic wire, elastics, bands, etc. If there are any cavities or food debris stuck within the teeth, then the dentist will clean the teeth and treat them accordingly. You need to maintain good oral hygiene during this stage to prevent complications such as dental decay, gum diseases, etc.

Debanding stage

Once you and your dentist are satisfied with your teeth positions, your braces will be removed. The dentist will remove residual bonding agents from the tooth surfaces and clean and polish the test. They will explain proper oral care to maintain the results of the treatment. They will also discuss your options for retainers and take impressions of your final teeth structures to fabricate the retainers.

Retainer Stage

Depending on your choice of retainers, your orthodontist will either fix your retainers on the lingual surfaces of your teeth or provide you with removable orthodontic appliances such as Hawley’s retainers, invisible resin retainers, Essix retainers, etc. If you have fixed retainers, you need to maintain oral hygiene around them and visit a dentist for consultation every 6-8 months to make sure it is not broken. If you have a removable retainer appliance, then you need to make sure you wear them as much as possible. Consult your dentist immediately if your appliance is broken or doesn’t fit.
