About Medical Termination of Pregnancy (MTP)

Medical abortion (MTP), as the name suggests, is to abort an unwanted pregnancy by a set of medicines. These medicines are commonly known as ‘RU486’ or ‘abortion pills’, and are typically considered a safe option to abort early pregnancies.

However, they must be taken only under strict medical supervision and followed with an ultrasound to ensure complete expulsion.

Abortion at home/ by your self / shady unregistered clinic could result in serious complications and result in unwanted side effects such as- incomplete abortion, extensive bleeding, tearing of the cervix, pelvic and urinary infection, nausea, fever, and chills. Statistically, more than 10 women die everyday because of unsafe abortion in India.

Therefore, if you are considering abortion, please consult a registered and experienced OBGYN at the earliest and let your doctor guide you through your best suited abortion method.

Diagnosis/ Eligibility

Medical Abortion is a safe and effective method to terminate early pregnancy. However, to ensure it’s the right method for you, the doctor must walk you through a few diagnostic imaging tests. This typically includes a pelvic ultrasound. This test is important to confirm both- the age and type of your pregnancy.

You are eligible for medical abortion only if:

  • Your weeks in pregnancy are lesser than 7-9 weeks.
  • Your pregnancy is not ectopic pregnancy. That is, pregnancy outside the uterus.

The doctor also checks for if you are anemic or have any other comorbidities, such as- diabetes, low/ high blood pressure. The cumulative results help decide the method best abortion method for you.

Procedure:

Medical abortion involves using a combination of two different medicines- mifepristone and misoprostol. These are commonly called “RU 486”.

The first medicine, ‘mifepristone’ blocks the necessary hormone for pregnancy and detaches the embryo from the uterine wall. It is given in the clinic. Then, you are asked to take the second set called- ‘misoprostol’. It causes the uterus to contract and opens the cervix slightly to expel the pregnancy. It is taken within 48 hours of your first medicine and can be taken at both- home/ clinic.

Typically, this initiates the expulsion within 24 hours. You might also experience a slight fever, abdominal cramps and nausea during this time. However, as you start passing the pregnancy, your fever would come down to normal and nausea would ease.

Note that passing of the pregnancy is usually very painful, prolonged, and marks extensive bleeding, cramping, and discomfort. It may take you a little over 7-10 days to expel the pregnancy completely.

Right after you pass the pregnancy, please arrange for a follow up. An ultrasound exam must be taken within 15 days to ensure complete abortion. In case the abortion fails, or turns uncomplete, surgical intervention is employed.

Recovery after medical abortion

Recovery is a crucial time period post-pregnancy. Make sure you:

The passing of the pregnancy in medical abortion comes with intense cramping. We suggest you use heat pads and gently massage downwards on your abdomen to help ease the pain and discomfort.

Passing of pregnancy may make you feel drowsy, weak, and nauseated. It is important you stay hydrated and eat a well-balanced diet. Therefore, we suggest you include lots of protein, iron, calcium and vitamin B. You can find these in green leafy vegetables, whole grains, fruits, dry fruits and milk.

As your body slowly recovers, we suggest you do light breathwork, yoga and stretching exercises. This should help ease the cramps and balance the emotions.

Abortion marks a period of a lot of hormonal and emotional changes in a woman. This is why you may feel weird, uneasy, anxious, and have multiple undefined feelings. However, please know that this is normal and only temporary. Therefore we suggest you spend some time relaxing, watching your favorite movie or talking to your loved ones- people you trust. Talking about your feelings, fear, goals, and desires with your friends and companion should help you feel positive and relaxed.

Birth control methods to prevent further unwanted pregnancy

Though the abortion ends the present pregnancy, you are likely to ovulate very soon and become fertile again. Therefore, to avoid an unwanted pregnancy in the future, it is important you switch to birth control immediately. You can choose from a variety of birth control methods depending on the regularity of your sexual activities and the desire for future childbirth. Some of the options are:

Barrier methods- These are short-acting methods and include condoms and daily contraceptive pills. You can become pregnant again as soon as you stop using them.

Contraceptive implants- These refer to the use of Intrauterine Devices(IUD). These are long-acting but reversible options. Once you decide to become pregnant again, you can ask your doctor to remove them, and you will be ready for conception again.

Permanent methods- These are ideal for women who have no desire for more childbirth. If so, you can opt between female sterilization or a male vasectomy. These are permanent and mark complete infertility.
