Retrograde intrarenal surgery or RIRS is an advanced procedure to remove kidney stones as large as 14mm. The surgery can help treat stones that are stuck in the kidneys (upper calyx, middle calyx, and lower calyx), ureter, or urinary bladder without the need for any cuts or incisions. RIRS uses a ureteroscope and advanced technology to remove stones with minimal complications. Get in touch with us to know more about the RIRS surgery in Bankura.
Who can benefit from RIRS Surgery?
RIRS is an effective procedure for patients with a history of recurring stones or when the stone size is too large to pass naturally. RIRS can provide relief from pain for stubborn stones that are usually immune to medications like diuretics. The surgery is performed inside the kidneys or ureter without the need for external cuts or incisions. RIRS surgery is likely to benefit patients with the following conditions -

Several diagnostic tests done before the RIRS treatment include the following –
Before the surgery, the patient is administered with spinal or general anesthesia. Spinal anesthesia sedates the lower half of the body. General anesthesia makes the patients unconscious (they will be asleep during the entire procedure). The choice of anesthesia is usually based on the preference of the patient.
After the anesthesia is given, the urologist inserts a long, flexible tube called an endoscope into the urethral passage to the ureter to reach the urine collecting part of the kidneys. The surgeon simultaneously also leverages x-rays and image screening to generate live images of the kidneys on an external screen for extreme precision. The endoscope is moved up in retrograde towards the kidneys.
Once the stones are located through the endoscope, the surgeon uses a laser probe to crush or manipulate the kidney stones accordingly. Then the stones are removed using small forceps in their intact form. We use an advanced holmium laser to target stubborn stones and break them without damaging the surrounding organs. The stone fragments are then subsequently collected or caught in the stone basket. Once the surgeon makes sure all the stone fragments are collected, the basket is removed.
The surgeon usually inserts double J stents to expand the ureteral passage. The stent is a flexible, hollow tube that runs from the kidneys to the ureter. The stent may be kept in the kidneys for as long as it takes for stones to completely flush out of the body. Usually, it is 10 to 14 days under normal circumstances. The objective of the stent is to enlarge the ureteral passage to help flush out stone fragments from the body smoothly. Further, the viability of the RIRS procedure has been upgraded by the progression and advancements in related tools and equipment like wires, ureteral access sheath, and stone containers.

Prolonging kidney stones can lead to possible complications –
Your doctor or healthcare institute generally provides instructions before the RIRS procedure to help you prepare for the surgery. Here is how you can prepare for your RIRS surgery –
After the RIRS procedure, the patient will be under the effect of anesthesia and may take time to regain consciousness completely. A catheter is usually placed for a day or two in the urethra to minimize the pain and discomfort while urinating. The patient will be shifted to the observational room, where the doctor will check the vitals and monitor your recovery. If the doctor does not suspect any possible complications, you may be allowed to go home. In case of any abnormality, you may be asked to stay in the hospital for observation purposes.
Recovery is an essential aspect of surgery. The normal recovery time after the RIRS procedure usually takes a day and the patient can resume work within two to three days after the surgery. Since there are no cuts and stitches, you can perform minimal physical activities that do not strain your lower body. Here are some recovery tips after the RIRS procedure –
RIRS is an advanced laser treatment that offers great results for patients experiencing immense pain. The surgery is done as an outpatient procedure which means the patient is usually discharged within 24 hours after the surgery if the doctor does not suspect any complications. In addition, RIRS offers a high stone passage rate in a single sitting as compared to the other treatment methods for kidney stone removal. Some of the advantages of RIRS are –
RIRS and PCNL both offer excellent results for large-sized kidney stones. However, for stones greater than 20mm in diameter, RIRS may not always produce satisfactory results. While RIRS is a great alternative to PCNL, it is observed that PCNL has a higher success rate for kidney stones of size 2-3cm in diameter. Retrograde intrarenal surgery or RIRS, however, only offers a comparable success rate for a stone size greater than 15mm. Some patients who undergo RIRS may regard it as a good alternative to PCNL. However, there are certain parameters that must be taken into consideration such as the age of the patient, location of the stone, prior history of open surgery, number of stones, degree of hydronephrosis, etc.
