Circinate balanitis is one of the most common types of balanitis which is a result of reactive arthritis. What Are The Symptoms Of Circinate Balanitis?

In the beginning, men with circinate balanitis experience pin-sized dots with a white plaque on them which grow into flat, red areas hardly covered by white plaque. Despite the visible symptoms of circinate balanitis, patients in nearly all cases do not experience any burning or itching, nor there is any strange smell. Due to its analogy to a fungal skin infection, circinate balanitis is often misdiagnosed as mycosis – specifically in cases where patients have no other symptoms of reactive arthritis.

Diagnosis of Circinate Balanitis

A doctor makes the diagnosis of circinate balanitis after carefully examining the skin. A small skin sample is taken from the skin of the penis and inspected under the microscope to confirm the presence of circinate balanitis. 

Treatment of Circinate Balanitis

Cortisone creams are the most commonly used treatment for circinate balanitis. Since reactive arthritis cannot be healed as such, patients are advised to follow a continuous topical treatment – otherwise, they might notice the symptoms again after 3-4 days of the treatment. 

Circinate balanitis, if recurrent, requires circumcision. Circumcision is the surgical removal of the foreskin covering the tip of the penis. In the absence of the foreskin, there is no longer an environment for an infection or inflammation that can result in circinate balanitis. Hence, an effective treatment for circinate balanitis can be open, laser circumcision and stapler circumcision

What Are The Possible Complications Of Circinate Balanitis?

Prolonged circinate balanitis can have certain risks and complications, if left untreated. Some of the complications of circinate balanitis include:

  1. Phimosis: Phimosis is a urological condition defined as the inability to retract the foreskin that covers the head of the penis. Phimosis may emerge as a tight ring of the foreskin, which circles the tip of the penis that prevents full retraction.
  2. Paraphimosis: Paraphimosis is a urologic emergency that happens in uncircumcised males when the foreskin gets trapped behind the corona of the glans penis. It can lead to strangulation of the glans, distal venous engorgement, and even necrosis.
  3. Ulcerative lesions: Circinate balanitis, if left untreated, can also lead to painful sores that bleed and ultimately cause scarring.
  4. Risk of Cancer: Chronic circinate balanitis increases the risk of various types of cancer. Quick treatment of circinate balanitis can significantly reduce the risk of complications.

  • You might experience pain after circumcision. Your urologist will prescribe medicines for pain management, and ensure you take all the medicines as instructed by the doctor.
  • Only wear loose-fitting clothes after the surgery and wear supportive underwear that would keep the penis protected.
  • Do not overexert yourself, and avoid strenuous physical activities for some time after the surgery.
  • Avoid harsh soaps or lotions that can irritate the skin.
  • Avoid having sex for at least 4 weeks after circumcision.
