Who Needs a Revisional Bariatric Procedure?

What Should I Expect After a Revision Bariatric Surgery?

Although results vary, most patients lose less weight with a revision than they did after their initial surgery. Additionally, your previous bariatric procedure will also affect your results. For instance, many patients who don’t lose much weight with an LAP-BAND lose weight after having their gastric bypass revised.

Types of Bariatric Surgery

The “Roux-en-Y” gastric bypass technique involves the surgeon’s cutting off a tiny pouch of the patient’s upper stomach, close to the esophagus, so that food can be stored there. After that, surgeons link a portion of the small intestine to the bottom of the newly formed stomach pouch.  Bypassing the majority of the stomach and a section of the small intestine, food passes via the small stomach pouch and into the small intestine when a patient eats. The tiny stomach pouch may grow or stretch out with time. Anti-obesity drugs, endoscopic procedures, or surgery can all be used to fix this.

Additionally, the gastric bypass can be converted to a single anastomosis duodenal-ileal bypass with sleeve gastrectomy (SADI).  To reverse the gastric bypass, the stomach is reattached, a gastric sleeve is performed, the previous small bowel is reattached, and a loop of the small bowel is attached to the end of the gastric sleeve.

  • Gastric Band Revision Surgery 

Doctors divide the upper part of the patient’s stomach into two portions by wrapping an adjustable silicone belt (or band) around it. This procedure is sometimes referred to as lap-band surgery.  With a band limiting their stomach size, patients experience fullness sooner than they otherwise would.

In the event that a patient has had gastric band surgery and the band has moved, the surgeon can just reposition the band. Sometimes, after removing the band for a few weeks, the surgeon replaces it because a poorly placed gastric band may have left scars or inflammation that need time to heal.  Surgeons may also choose to perform the SADI operation, gastric bypass surgery, or sleeve gastrectomy instead of gastric band surgery.

During this surgery, a large section of the patient’s stomach is surgically removed, reducing the stomach’s capacity by 80%.  This makes the stomach appear less like a pouch that can store and aid in the digestion of food and more like a tube through which food passes.  

Weight gain may result from the stomach becoming larger or stretched out after a patient undergoes a sleeve gastrectomy.  Surgeons usually convert sleeve gastrectomy to gastric bypass or a duodenal switch treatment rather than creating a smaller stomach sleeve. Another good way to treat gastroesophageal reflux disease(GERD) symptoms that might have developed after a sleeve gastrectomy is to switch to a gastric bypass.
