What is Lower Ureteric Calculus?

Lower ureteric calculus or ureteral calculus refer to ureteric stones in the lower part of the ureters. Ureters are two tubes responsible for carrying urine from your kidneys to the urinary bladder. Both kidneys have one ureter each. Ureteral stones are solid or hard deposits that get stuck in the ureters. The ureteric calculus can differ in their composition. The most common ureteric stones include calcium oxalate, calcium phosphate, cystine, struvite, and uric acid stones.

Ureteral calculus can exhibit different symptoms, which can be pretty discomforting and affect the patient’s quality of life. In addition, untreated ureteral stones can even lead to potential complications such as renal abscess, stone infections, chronic kidney disease (CKD), ureteral scarring, obstruction, and stenosis. Therefore, it is advised that patients seek timely treatment for these stones.

How is Lower Ureteric Calculus Diagnosed?

The diagnostic procedure for ureteric calculus includes some standard tests. But before suggesting these tests, the doctors perform a thorough physical examination of the patient’s condition. This assessment consists of understanding the patient’s symptoms and their medical history. After performing a physical examination, the doctor will then suggest a series of tests to check how serious the condition is and determine the exact location and size of the stones. Some standard tests include:

  • Urinalysis: A urinalysis is performed to rule out the presence of any infections in the urine and check substance levels in the urine that may have caused lower ureteric calculus to form.
  • Blood test: Blood tests help determine the type of stone the patient has. These tests can also help rule out kidney function and other problems that may have caused the stones to form.
  • Ultrasound: In an ultrasound, sound waves are used to look for blockages in the ureters and determine any other underlying conditions.
  • Computed tomography (CT) scan: A CT scan is another imaging test that helps determine the stone’s location, size, etc.

Are you going through any of these symptoms?

What are the Treatment Options for Lower Ureteric Calculus?

Doctors prepare a specialized treatment plan for every patient after the diagnostic process is complete and reports are acquired. The choice of which treatment would be the most suitable for the patient depends on the findings of the diagnostic process and the patient’s overall health. Here are the most common treatment options for upper ureteric stones:

Medicine & therapy: When the stones are small, and there are no complexities, medicine and therapy work just fine in relieving the symptoms and helping stones pass through the system. The doctors may prescribe pain relievers to manage the pain caused while passing the stone. Or they may also suggest alpha-blockers which help relax the muscles in the ureters and help the stone pass easily. In addition, the doctor may also suggest some lifestyle changes to help the stones pass and prevent future stone formation.

Other treatments include:

  • Eswl

Extracorporeal Shock Wave Lithotripsy is a commonly performed treatment procedure for removing ureteric calculus. It is typically suggested for small stones. This is a non-surgical procedure that involves no cuts or stitches. The doctors use sound shockwaves through the flank area, with X-ray guidance, to break the stones into smaller fragments, allowing them to pass much more quickly. This procedure is generally considered effective for stones that are smaller than 1 cm in size.

  • Ureteroscopy with laser lithotripsy

In a ureteroscopy, the doctors use a ureteroscope to perform the surgery. It is a thin tube with a camera at the end, which is inserted through the urethra, bladder, and then into the ureter. The ureteroscope gives a visual of the stone and helps doctors figure out the exact location of the stone. Once the stone is located, they use a laser to reduce the stones to smaller fragments. The patients can easily pass these fragments out of the body as they urinate.

  • Open stone surgery

Open stone surgery is a traditional method of removing lower ureteric calculus. In this method, the surgeon makes an incision in your side or the lower back to access your kidney or ureter and then uses surgical equipment to extract the stone. This procedure becomes necessary if the stones are too large in size or if the patient has a skeletal deformity. This procedure is not commonly performed these days and accompanies a higher risk of complications compared to the new, less-invasive methods of stone removal. The recovery time is also much higher with open surgery.

How to Prepare for Lower Ureteric Calculus Treatment?

Preparation is key before a lower ureteric calculus treatment to ensure a seamless treatment journey. Your doctor will give you instructions to help you prepare for the treatment. Some general guidelines to prepare for the treatment are as follows:

  • Before surgery, make sure you observe a fast for 7 to 8 hours or as directed by the doctor. Eating anything during this period can cause complications with anesthesia.
  • Ensure your body is thoroughly hydrated by drinking ample fluids in the days leading up to the surgery. This will help toxins build up in the body and help you have a smooth treatment journey.
  • Talk to your doctor about which medicines to discontinue before the surgery. This is important because certain medications, such as aspirin or blood thinners, can increase your risk of bleeding.
  • Prepare your house before you go for surgery, as you will be asked to rest entirely in the days following the procedure.
  • Ask your doctor any questions you may have about the procedure to remain calm during the process.
  • If your doctor has given you any specific instructions to prepare for the surgery, make sure you follow them diligently to avoid complications later on.

What is the Recovery Period After Ureteric Calculus Treatment?

The recovery period after upper ureteric calculus treatment can differ depending on the type of treatment you’ve received. The total recovery duration is different for all procedures and may also vary depending on the patient’s overall health and whether there were any complications in the treatment process.

In the case of the ESWL procedure, patients can return home the same day. Total recovery can take 1 to 2 days.

After a Ureteroscopy with laser lithotripsy, the patients may have to spend a night in the hospital after the URSL procedure. Complete recovery time can range from 4 to 7 days following the procedure.

If the patient has had an open surgery, they can expect a longer recovery period. It may take the patient around 6 weeks to recover fully and get back to their routine.

During the recovery period, patients can do a range of things to promote healing and recover more quickly. This can include making some dietary and lifestyle modifications and following the after-care instructions provided by the doctor diligently. Here are some things you can do to boost your recovery after upper ureteric calculus treatment:

  • Stay hydrated at all times. Drink at least 8 to 10 glasses of water a day.
  • Do not consume too much sodium and oxalate. Eat a balanced diet that is rich in fruits, vegetables, and whole grains.
  • Limit your alcohol and caffeine consumption to prevent dehydration. It can irritate your bladder and lead to further discomfort.
  • Don’t indulge in strenuous activities immediately after the surgery. Resume exercise gradually, and start with low-impact exercises such as walking, swimming, and yoga.
  • Take the prescribed medicines on time. Also, talk to your doctor before discontinuing any medicine or starting a new one.
  • Take care of the incision (if any) by maintaining proper hygiene and taking care of the wound. Get your dressing changed on time.
  • If you experience pain, discomfort, or other complications, make sure you immediately consult with your respective healthcare provider.
  • Avoid stressing yourself out, as it may impact your immune system and increase the risk of renal concretions recurrence. It may also delay healing and increase your recovery time.

Frequently Asked Questions

How much will lower ureteric calculus treatment cost me in India?

The cost of lower ureteric calculus treatment in India differs for all patients. However, on average, the treatment can cost you anywhere between Rs.40,000 and Rs. 2,00,000. This cost is subject to differ, chiefly on the basis of the type of treatment the patient is receiving. Here’s a bifurcation of the estimated treatment cost based on the type of treatment:

  • ESWL: Rs 40,000 to RS. 60,000.
  • Ureteroscopy: Rs. 50,000 to Rs. 90,000.
  • Open Stone Surgery: Rs. 1,00,000 to 2,00,000

What factors will affect the cost of my lower ureteric calculus treatment in India?

Several factors can impact the total expense you may have to bear for your lower ureteric calculus treatment in India, including the city you’re receiving your treatment, your choice of treatment hospital, the type of treatment, the severity of your condition, the cost of anesthesia, cost of any additional drugs, cost of diagnostic tests, and so on.

Which size of lower ureteric calculus is harmful?

Large stones pose the risk of ureteral blockage as they do not pass through the system on their own. Typically, lower ureteric calculus larger than 1 cm in size is considered dangerous and may lead to complications if not treated in time.

Is treatment always required for lower ureteric calculus?

If the stones are small in size, there’s a chance they can pass naturally through the system. In the case of small stones, even home remedies can help in the removal of calculus. However, when the stones are large or there’s a risk of obstruction, treatment becomes necessary.

Can lower ureter calculus harm the kidneys?

Yes. In some cases, lower ureteral calculus can lead to an obstruction in the kidneys and cause serious complications, including urinary tract infections, kidney damage, and complete renal failure. To ward off these complications, the doctors suggest seeking prompt treatment.

What causes lower ureteric stones?

Lower ureteric stones are often associated with a family history of stones. Patients with a history of stones in their families are more likely to develop stones. In addition, some metabolic disorders can also put patients at an increased risk of developing ureteric calculus, such as cystic kidney diseases and urinary tract infections. Some lifestyle habits, such as insufficient consumption of water, consuming unhealthy foods, etc., can also increase a person’s risk of developing lower ureteric stones.

How will I know I have lower ureteric stones?

To determine whether they have lower ureteric stones, the patients first need to recognize the symptoms of this condition. Sometimes, ureteric calculus is too small to exhibit any symptoms. But, when the stones are large, multiple, or are obstructing the ureters, the patient may experience severe symptoms such as, pain in the flank area that spreads to the groin, frequent and painful urination, blood in the urine, a burning sensation while urinating, fever, and chills. If an individual experiences these symptoms, it is important to seek prompt medical attention. The doctor will then perform some tests to confirm the presence of the stones.

What are the chances of lower ureteric stone recurrence after treatment?

You should note that treatment doesn’t help with the prevention of stones from occurring. There’s always a chance of recurrence in the case of ureteric stones, and therefore, patients must take the necessary precautions to prevent stones from recurring. Prevention includes making some lifestyle changes, such as keeping hydrated, eating healthy, cutting back on junk and processed foods, etc.

When will I be able to go home after my ureteric calculus treatment?

The duration of hospitalization after ureteric calculus treatment depends on the type of treatment you’ve received. While some treatments for lower ureteric stones, such as ESWL, do not require a hospital stay at all, other treatments, like open stone surgery or ureterorenoscopy, may require a hospital stay of a day or more. Other factors also impact the need for a hospital stay, including the patient’s overall health following the surgery, whether there were any complications during or after surgery, and so on.

How to prevent lower ureteric calculus in the first place?

There are various things a person can do to prevent lower ureteric stones in the first place, including:

  • Keeping hydrated by drinking ample, healthy fluids throughout the day.
  • Limiting the consumption of animal protein in the diet.
  • Cutting back on sodium or salt in the diet.
  • Limiting the amount of oxalate in the diet as it may also lead to the formation of stones.
