Overview

Open appendectomy involves surgical removal of the appendix through a conventional abdominal incision. The appendix is a small finger-shaped pouch attached to the large intestine. When infected, the condition is called appendicitis, which requires urgent intervention. Open surgery provides direct access and is preferred when the appendix has burst or when adhesions are present from prior surgeries.

Causes

Common causes of appendicitis requiring open surgery include:

  • Blockage of the appendix by hardened stool or foreign objects
  • Bacterial or viral infections spreading to the appendix
  • Inflammatory bowel disease causing inflammation
  • Parasitic infections causing obstruction
  • Enlarged lymph nodes pressing on the appendix
  • Trauma or injury to the abdomen

Are you going through any of these symptoms?

Symptoms

Symptoms of appendicitis that may require open surgery include:

  • Sudden severe pain in the lower right abdomen
  • Nausea and vomiting
  • Fever and chills
  • Loss of appetite
  • Abdominal bloating and rigidity
  • Diarrhea or constipation

Types of Appendicitis

Appendicitis is classified as acute, where symptoms develop suddenly over 24 to 72 hours, or chronic, where mild symptoms persist for weeks. Perforated appendicitis occurs when the appendix ruptures and spills infection into the abdominal cavity, and is the most serious form requiring open surgery. Recurrent appendicitis involves repeated mild episodes that may eventually need surgical intervention.

Who Needs Open Appendix Surgery?

Open appendectomy is recommended when the appendix has ruptured and caused widespread infection, when laparoscopic surgery cannot be completed safely, when the patient has significant scar tissue from prior surgeries, or when access to laparoscopic equipment is limited. The surgeon evaluates the clinical condition thoroughly before deciding the appropriate surgical approach.

Diagnosis

Diagnosis of appendicitis involves:

  • Physical examination to check for tenderness at the lower right abdomen
  • Blood tests to detect elevated white blood cell count indicating infection
  • Urine tests to rule out kidney stones or urinary tract infection
  • Ultrasound of the abdomen to visualize the appendix
  • CT scan of the abdomen and pelvis for detailed imaging
  • MRI scan recommended for pregnant patients

The Alvarado score is also used to estimate the likelihood of appendicitis based on clinical findings.

Surgical Procedure

Open appendectomy is performed under general anesthesia. The surgeon makes a 2 to 4 inch incision in the lower right abdomen, separates the abdominal muscle layers, locates the appendix, ties off its blood supply, and removes it. The area is cleaned and the incision is closed in layers. The procedure typically takes 30 to 60 minutes depending on severity.

Open Appendectomy Steps

The open appendectomy procedure involves:

  • Administration of general anesthesia to keep the patient asleep and pain-free
  • Making a 2 to 4 inch incision at the right lower quadrant of the abdomen
  • Separating the muscle and tissue layers to access the abdominal cavity
  • Identifying and isolating the inflamed appendix
  • Ligating the base of the appendix and dividing the mesoappendix
  • Removing the appendix and inspecting for additional infection
  • Irrigating the abdominal cavity if rupture has occurred
  • Closing the incision in multiple layers using absorbable sutures

After the Surgery

Recovery after open appendectomy takes 4 to 6 weeks. Patients are discharged within 2 to 3 days for uncomplicated cases, or longer if the appendix had ruptured. Patients should avoid strenuous activity and heavy lifting for at least 4 weeks. A soft diet is recommended initially. Regular wound care, pain management, and follow-up visits are important for smooth recovery.

Complications of Open Appendix Surgery

Possible complications after open appendectomy include:

  • Wound infection at the incision site
  • Formation of intra-abdominal abscess
  • Ileus or temporary stoppage of bowel movement
  • Hernia at the incision site in rare cases
  • Internal bleeding or hematoma formation
  • Adverse reaction to anesthesia
  • Deep vein thrombosis
  • Injury to nearby organs such as the bladder or bowel

FAQs About Open Appendix Surgery

Is open surgery needed?

Open surgery is needed when the appendix has ruptured, when laparoscopic surgery is not feasible, or when scar tissue from prior surgeries is present. The surgeon determines the best approach based on the individual condition and clinical assessment.

How long is recovery after open surgery?

Recovery after open appendectomy takes 4 to 6 weeks. Most patients are discharged within 2 to 3 days. Avoid heavy lifting for 4 weeks and gradually resume activities as advised by the surgeon. Full recovery is expected within 6 weeks for most patients.

Can I eat normally post-surgery?

After surgery, start with clear liquids and gradually move to soft foods. Most patients resume a normal diet within 1 to 2 weeks. Avoid spicy and greasy foods initially to support digestive recovery.

What are signs of infection after surgery?

Signs of infection include redness, swelling, or discharge at the incision site along with fever. Contact your surgeon immediately if these symptoms appear for prompt evaluation and appropriate treatment.
