Gestational Diabetes

Gestational diabetes mellitus (GDM) is a type of diabetes that develops during pregnancy in women who did not previously have diabetes. It occurs when the body cannot produce enough insulin to meet the increased demands of pregnancy, leading to elevated blood glucose levels. If uncontrolled, GDM can cause complications including large birth weight, preterm birth, and increased risk of type 2 diabetes later in life. Pristyn Care gestational diabetes clinics offer expert screening, diagnosis, and personalised management.

Symptoms

Gestational diabetes often has no obvious symptoms, but signs may include:

  • Increased thirst and frequent urination
  • Fatigue beyond normal pregnancy levels
  • Blurred vision
  • Frequent infections such as thrush or urinary tract infections
  • Nausea beyond the first trimester
  • Excessive hunger despite eating

Are you going through any of these symptoms?

Causes

Gestational diabetes develops due to:

  • Hormonal changes during pregnancy that impair insulin function
  • Increased insulin resistance from pregnancy hormones including progesterone and human placental lactogen
  • Inadequate insulin production to meet increased demands
  • Genetic predisposition and family history of type 2 diabetes
  • Obesity or being overweight before pregnancy
  • Previous history of gestational diabetes

Types of Gestational Diabetes

Gestational diabetes is classified by severity and treatment:

  • Class A1 GDM – controlled with diet and exercise alone
  • Class A2 GDM – requires insulin or oral medication in addition to dietary control
  • Pre-gestational diabetes – diabetes present before pregnancy that persists during gestation

Risk Factors

Risk factors for developing gestational diabetes include:

  • Pre-pregnancy obesity (BMI over 30)
  • Family history of diabetes
  • Previous gestational diabetes
  • Age over 35 years
  • Previous delivery of a baby weighing over 4.5 kg
  • Polycystic ovary syndrome (PCOS)
  • South Asian, Middle Eastern, or African ethnic background

Who Is at Risk?

Women who are overweight, have a family history of diabetes, are over 35, or had gestational diabetes previously are at highest risk. Women with PCOS or from certain ethnic groups including South Asian and Middle Eastern backgrounds also face elevated risk.

Diagnosis

Diagnosis of gestational diabetes involves:

  • Fasting blood glucose at booking and at 24-28 weeks of pregnancy
  • Oral glucose tolerance test (OGTT) measuring fasting, 1-hour and 2-hour glucose levels
  • HbA1c at booking to detect pre-existing diabetes
  • Random blood glucose testing if symptomatic
  • Continuous glucose monitoring in high-risk cases

Treatment Options

Treatment starts with dietary modification and regular moderate exercise. Most women achieve adequate glucose control with lifestyle changes alone. If targets are not met, metformin or insulin therapy is added. Blood glucose is monitored regularly with a home glucometer. Delivery is typically planned at 38-40 weeks or earlier if complications arise.

Gestational Diabetes Management

The gestational diabetes management program at Pristyn Care includes:

  • Personalised meal plan by a specialist dietician
  • Structured exercise guidance adapted for safe use during pregnancy
  • Self-monitoring of blood glucose with a home glucometer
  • Regular antenatal ultrasound scans to monitor fetal growth
  • Metformin or insulin therapy if lifestyle measures are insufficient
  • Fetal wellbeing assessment with non-stress tests and Doppler studies
  • Coordination between diabetologist, obstetrician, and dietician
  • Delivery planning based on glucose control and fetal size

Our multidisciplinary team ensures optimal glucose control and a healthy outcome for mother and baby.

After Delivery Care

Postnatal care after gestational diabetes includes:

  • Blood glucose monitoring after delivery to confirm resolution
  • Glucose tolerance test at 6-12 weeks postpartum to screen for type 2 diabetes
  • Breastfeeding encouraged as it reduces long-term diabetes risk
  • Dietary and lifestyle counselling to reduce future diabetes risk
  • Annual fasting blood glucose or HbA1c monitoring long-term
  • Advice on weight management and physical activity

Possible Complications of Gestational Diabetes

Untreated or poorly controlled gestational diabetes can cause:

  • Macrosomia – large birth weight baby increasing delivery complications
  • Shoulder dystocia during vaginal delivery
  • Increased risk of emergency caesarean section
  • Neonatal hypoglycaemia in the newborn
  • Premature birth and neonatal intensive care admission
  • Pre-eclampsia and hypertension in the mother
  • Increased risk of type 2 diabetes in the mother later in life

Frequently Asked Questions (FAQs)

What is gestational diabetes?

Gestational diabetes is a type of diabetes that develops during pregnancy in women without pre-existing diabetes. It occurs when pregnancy hormones cause insulin resistance and the body cannot produce enough insulin to maintain normal blood glucose levels.

Can gestational diabetes harm my baby?

If uncontrolled, it can lead to a large baby, birth complications, neonatal hypoglycaemia, and increased risk of childhood obesity. With proper management, risks to both mother and baby are significantly reduced.

Does gestational diabetes go away after birth?

In most women, blood glucose returns to normal after delivery. However, women who had gestational diabetes have a significantly higher lifetime risk of developing type 2 diabetes and should have annual screening.

What diet is recommended for gestational diabetes?

A gestational diabetes diet limits refined carbohydrates, distributes carbohydrate intake evenly across meals, increases fibre and protein, and avoids sugary foods and drinks. A specialist dietician provides a personalised meal plan.

I recently underwent hernia surgery under Dr. Abhilash Madhavan, and I am extremely satisfied with the treatment. The doctor explained the condition, surgery procedure and recovery plan very clearly, which gave me full confidence before the operation. The surgery went smoothly and my recovery has been excellent. Till the bandage removal, the support and follow-up from the doctor and medical team were very good. I would like to give special appreciation to Mr. Arun from Pristyn Care – Medical Counsellor. He guided me from the very beginning, answered all my queries with patience and made sure I was mentally comfortable before and after the surgery. His continuous support truly made the experience easier. Overall, I am very happy with the outcome of my surgery. My sincere thanks to Dr. Abhilash Madhavan, Mr. Arun from Pristyn Care, and the entire team for their dedication and care. I definitely recommend them for hernia treatment.
