How Does A Semen Analysis Work?

A semen analysis involves a series of steps to evaluate various characteristics of semen. Here’s how it typically works:

  • Collection of semen sample: The first step is collecting a semen sample. The sample can be collected at home or in the clinic. If you are collecting a sample at home. Keep it at room temperature and get it to your doctor or clinic within one hour. Don’t use lubricants when you collect the sample because they can impact how easily your sperm can move around. If the sample will be collected in the clinic, avoid ejaculation for at least 2 to 3 days before the test for exact results. Also, for accurate results, the semen must be examined quickly before the sperm begins to die.
  • Sample processing: Once the sample is collected, it is brought to the laboratory and allowed to sit briefly (20 to 30 minutes) to undergo liquefaction. Normal semen is thick and gel-like immediately after ejaculation but should become more liquid within 20 to 30 minutes.
  • Sperm examination: A small amount of the liquefied semen is then placed on a glass slide by the lab technician to assess its quality and quantity. The semen sample assessment helps count the number of sperm present in a specific sample volume. This provides the sperm count. Another portion of the liquefied semen is examined to evaluate the movement of the sperm. The technician also examines the shape and structure of the sperm to identify any abnormalities. Additional tests may be performed to check for the presence of white blood cells, fructose content, and other factors that can affect fertility.
  • Analysis of results: The results are then analyzed and reported, typically including values and references for each parameter. These results are compared to established reference ranges to determine whether they fall within the normal or abnormal range.

What Are The Parameters That Are Evaluated In Semen Analysis?

Once you’ve provided your semen sample, it will be examined under a microscope to evaluate the following parameters:

1. Sperm count (Concentration): Concentration estimates the number of sperm in one milliliter of semen. A healthy sperm count is generally considered 15 million to greater than 200 million sperm per milliliter of semen. Low sperm count can indicate problems with sperm production or delivery.

2. Semen volume: Semen volume is one of the main parameters of semen analysis. The normal volume of semen secreted during an orgasm is around 1.5 ml to 5.5 ml. A reduction in the semen volume may happen in case of retrograde ejaculation,  ductal obstruction, congenital irregularities or absence of the ducts or seminal vesicle, neurological issues, hormonal imbalance, etc. 

3. Motility: The sperm moves with the help of a flagellum, also known as the tail of the sperm. The sperm typically moves from 1 mm/min to 4 mm/min. The normal motility of the sperm is crucial for the transportation of the sperm through the female genital tract and successful fertilization of the ovum. The grading of sperm motility is as follows :

  • Grade 0 refers to no movement
  • Grade 1 is slow movement
  • Grade 2 is a sluggish but not straight movement
  • Grade 3 is movement in a straight line
  • Grade 4 is a good speed

It is considered normal if more than 60% of the sperm in a semen sample tested show normal motility. Abnormal sperm motility occurs in men with partial duct obstruction, testicular infections, or structurally abnormal sperms.

4. Morphology: Sperm morphology refers to the anatomical characteristics of sperm, such as its shape and size. The different parts of the sperm, such as the head, acrosome, mid-piece, tail, etc, are analyzed in detail under a phase contrast microscope after staining with a special papanicolaou stain. Normal sperms have an oval head and are 3-5 micrometers long and 2-3 micrometers wide. Mature sperms may contain some droplets of cytoplasm in the mid-piece, but undue retained cytoplasmic droplets may be abnormal. Around 200 to 250 sperms are examined during semen analysis, out of which more than 70% of the sperms in a semen sample should be of normal morphology, and less than 2 to 3 % should be immature forms.

5. pH: The pH level of semen measures its acidity or alkalinity. A slightly alkaline pH (around 7.2 to 8.0) is considered normal. Variations from this range can impact sperm survival.

6. Liquification: Normal semen comes out thick during ejaculation. Liquefaction time estimates how long it takes before it becomes liquid. It should take about 20 to 25 minutes. If yours takes longer or doesn’t become liquid, it could indicate a problem.

7. White blood cells: The presence of white blood cells in semen can mean infection or inflammation in the male reproductive system.

8. Appearance: The appearance of the semen should be whitish to gray and opalescent. A semen with a red-brown tint could show the presence of blood, while a yellow tint could mean jaundice or be a medication side effect.

9. Fructose level: Fructose is a sugar produced by the seminal vesicles. Its existence in semen provides energy for sperm. A low fructose level can indicate a problem with the seminal vesicles.

10. Viscosity: Viscosity refers to the stickiness or thickness of semen. Normal semen should become less viscous during liquefaction. High viscosity can hinder sperm movement.

These parameters are collectively evaluated to determine the overall quality and health of sperm. A thorough analysis helps diagnose potential causes of male infertility and guides appropriate treatment options if required.   • Testosterone: Supplements that stimulate this hormone can transmit the wrong signal to the body and make it stop producing natural testosterone and sperm. It can lead to a low count or lack of sperm.
  • Anabolic steroids: Used to build muscle, these medications can affect the body’s sperm production.
  • Marijuana: The THC (tetrahydrocannabinol) in marijuana can hamper your sex drive and keep your body from producing testosterone and sperm the way it should.
  • Opiates: Opiates can lower the sperm count and impact testosterone production.

5. Wash your hands properly before producing the semen sample.

6. If you choose to produce a sample from the comfort of your home, ensure that the sample is kept warm and delivered to the fertility center or clinic within exactly one hour of ejaculation. 

7. Follow any specific instructions provided by your healthcare provider. This may include collection techniques and any additional information.

8. Remember to discuss any concerns or questions you have with your healthcare provider before the test.

  • Hormone testing: In hormone testing, the patient gives a blood sample. The blood sample undergoes a series of tests to obtain a complete hormone profile. Male hormonal testing evaluates the levels of testosterone, FSH, LH, and, in some cases, prolactin, which all work together to regulate sperm production, sexual function, and sex drive. 
  • Ultrasound: An ultrasound of the testicles or other reproductive organs may be performed to check for structural irregularities, such as varicoceles (enlarged veins in the scrotum) or blockages.
  • Genetic testing: DNA testing or genetic testing helps identify changes in DNA sequence or the structure of the chromosome. The confirmation of the suspected genetic condition comes from a genetic test, which also determines the chance of developing and passing any disease in the future. This test is typically recommended to identify any genetic factors contributing to infertility or abnormal semen parameters.
  • Infectious disease testing: If an infection is suspected in your semen analysis results, tests may be conducted to identify and treat any underlying infections that could affect sperm quality.
  • Hemizona assay test: A hemizona assay, also known as the hemizona binding assay, is a test used to evaluate the ability of sperm to bind to the zona pellucida, a protective layer surrounding the egg. This test is often used to further assess male fertility in cases where standard semen analysis may not provide a complete picture. It helps determine if sperm can bind to the zona pellucida, which is a vital step in fertilization. The results of this test can provide more information about male fertility potential and may be used in cases of unexplained infertility.
