Kidney Stone Treatment
Understanding the Condition

What Are Kidney Stones?

Kidney stones (renal calculi) are solid crystal-like deposits formed from minerals and salts in the urine. They develop when urine becomes concentrated, allowing minerals to crystallize and stick together.
Kidney stones vary significantly in size, shape, and composition, and the stone size plays a crucial role in determining:
  • Severity of symptoms
  • Chances of passing naturally
  • Need for medical or surgical intervention
  • While very small stones may pass without treatment, larger stones often get stuck in the urinary tract, causing intense pain and complications.

Did You Know?

Kidney stones larger than 7 mm rarely pass naturally and usually require medical procedures or surgery for complete removal.

How are Kidney Stones Diagnosed?

A Urologist diagnoses kidney stones using a combination of:

1

Medical History:

Evaluation of symptoms such as flank pain, burning urination, blood in urine, nausea, or recurrent UTIs.
2

Imaging Tests:

Ultrasound, CT scan (most accurate), X-ray (for certain stone types)
3

Stone Size Measurement:

Imaging helps determine the exact size (in mm) and location of the stone, which directly influences treatment planning.
4

Pre-Treatment Health Assessment:

Blood and urine tests may be advised before any surgical intervention.

Symptoms of Kidney Stones

The symptoms of kidney stones of different sizes are more or less the same. However, the intensity of these symptoms can differ.

How is Kidney Stone Surgery Performed?

A step-by-step guide to our advanced laser lithotripsy procedure

Step 1

Anesthesia

Anesthesia General or spinal anesthesia is administered to ensure the patient is completely comfortable and pain-free throughout the procedure.
Step 2

Sterilization & Device Placement

Sterilization & Scope Placement The urinary tract is cleaned, and a ureteroscope is carefully inserted through the urethra and bladder to reach the stone's location in the ureter or kidney.
Step 3

Stone Fragmentation

Stone Fragmentation Once the stone is located, the laser is activated to break it down into tiny fragments or dust in a precise, controlled manner, allowing easy passage through the urinary tract.
Step 4

Clearance & Dressing

Clearance & Dressing The stone fragments are flushed out or removed, a ureteral stent may be placed temporarily to aid drainage, and the patient is shifted to recovery and discharged within a few hours.

RIRS vs ESWL for Kidney Stones

While ESWL (Extracorporeal Shock Wave Lithotripsy) is often preferred for its non-invasive approach, RIRS (Retrograde Intrarenal Surgery) remains a reliable alternative when ESWL is less effective for larger or harder stones, complete stone clearance in a single session is desired, or the surgeon recommends RIRS based on stone location and anatomy. Your urologist will guide you in choosing the most suitable technique based on your condition.

Benefits of Kidney Stone Treatment

check

Health Benefits of Kidney Stone Removal

Generally, there are two most common types of patients for kidney stone treatment: adults with acute kidney stone pain or blockages, and patients with recurrent stones requiring preventive intervention. Currently, according to most research studies, the benefits of timely kidney stone removal outweigh the risks. Treating kidney stones reduces the risk of urinary tract infections, kidney damage, chronic kidney disease, and complete urinary blockage. Early treatment can even lower the risk of developing long-term renal complications and hypertension.
check

Modern Kidney Stone Treatment Techniques

IIn the past, open surgery (nephrolithotomy) was the norm for both these types of patients, but nowadays, laser lithotripsy and ESWL (Extracorporeal Shock Wave Lithotripsy) are preferred. Of these two techniques, laser lithotripsy (RIRS/URSL) is often preferred as it provides better results with minimal complications.
check

Advantages of Laser Lithotripsy

Laser lithotripsy preserves the surrounding kidney tissue and breaks down stones with minimal blood loss. It has a short recovery period and the patient is completely recovered within a couple of weeks. The minimally invasive approach prevents post-surgical infections and aids the smooth recovery of the patient.

Cost of Kidney Stone Treatment in India

The cost varies depending on:
  • Stone size and number
  • Type of procedure
  • Hospital and city
  • Insurance coverage
,Approximate Cost Range:
  • ESWL: ₹50,000 – ₹75,000
  • URSL / RIRS: ₹1,00,000 – ₹1,40,000
  • PCNL: ₹90,000 – ₹1,10,000
Most procedures are covered under insurance if medically required.

Kidney Stones That Can Pass Naturally

Here are the different sizes of kidney stones that have a chance to pass naturally.

Kidney Stone SizeChances of passing naturally (in %)
<2 mm Kidney StoneAlmost 85%
2 -4 mm Kidney StoneAlmost 80%
4 mm Kidney StoneAlmost 80%
4 – 7 mm Kidney StoneAlmost 60%
Larger than 7 mm Kidney StoneAlmost 30%
1 -2 cm Kidney StoneCannot be passed naturally
2 cm or above Kidney StoneCannot be passed naturally

Treatment for Small-Sized Kidney Stones (Up to 4 mm)

Small kidney stones often pass naturally with:
  • Increased fluid intake
  • Low-salt and high-fiber diet
  • Pain relievers
  • Medications to ease stone passage
Surgery is usually not required at this stage.

Treatment for Medium-Sized Kidney Stones (4–10 mm)

Medium-sized 4 mm stone in kidney to 10 mm can be treated through home remedies and conservative methods such as medication (alpha-blockers and painkillers). However, in complex cases, when the stones don’t respond to conservative treatment, the doctors may suggest some medical procedures or minimally-invasive surgeries. These may include:
  • ESWL or Extracorporeal Shock Wave Lithotripsy: This is a non-invasive procedure for kidney stone removal. In this method, the surgeon uses high-energy sound waves to break the stones into smaller pieces to allow them to flush out of the system on their own. This procedure only takes 30 to 45 minutes, and the patient is allowed to go back home on the same day.
  • URSL or Ureteroscopic Lithotripsy: This procedure involves extracting the kidney stones with the help of a ureteroscope i.e., a long fiber-optic scope. This scope is inserted into the ureteral passage through the urethra, and an x-ray is simultaneously performed to determine the location of the stones. On locating the stones, the doctor uses a stone basket to collect the stones and remove them from the system. If the stones are too big in size, the surgeon may use a laser to break them into small pieces, which are allowed to flush out of the system naturally. A stent is inserted to widen the passage of the ureter, allowing stones to pass easily.

Treatment for Large Kidney Stones (Larger Than 10 mm)

Treatment for large-sized kidney stones (10 mm stone in kidney or more) typically includes surgery. Some common surgeries include:
  • RIRS or Retrograde Intrarenal Surgery: This ureteroscopic technique of extracting kidney stones involves reaching the kidneys through the urethra. It is a minimally invasive procedure i.e., it involves no cuts or stitches. An optic endoscope reaches the kidneys, which locates the stones. Then, a laser is used to reduce the stones to dust. This procedure is generally suggested for the treatment of large and complex kidney stones.
  • PCNL or Percutaneous Nephrolithotomy: This surgical procedure for kidney stones treatment is suggested when other techniques fail to offer any results. In this method, a small hole is made in the flank area for the removal of the stones. The stones are then removed with the help of microforceps or are reduced to dust with the help of a laser. When the stones are removed, doctors use contrast to confirm that no pieces are left in the system.

Types of Kidney Stones (Based on Composition)

  • Calcium stones (most common)
  • Uric acid stones
  • Struvite stones
  • Cystine stones
Although composition matters, treatment decisions are primarily driven by stone size and location.

Recovery After Kidney Stone Surgery

Recovery depends on the procedure:
  • ESWL / URSL: 2–5 days
  • RIRS: 5–7 days
  • PCNL: 7–14 days
Patients are advised to:
  • Stay hydrated
  • Follow dietary recommendations
  • Attend follow-up visits

What If Kidney Stones Are Left Untreated?

Untreated kidney stones can lead to:
  • Severe pain episodes
  • Urinary infections
  • Kidney swelling
  • Permanent kidney damage
  • Sepsis in extreme cases
Early diagnosis and size-based treatment prevent complications.

Frequently Asked Questions

Find answers to common questions about this treatment. Still have questions? We’re here to help!

Yes. Kidney stones can vary significantly in terms of their size. They can be as small as the size of a grain of sand grain and as big as the size of a golf ball. Several factors can cause the size of kidney stones to increase, including consuming too much salt, having a high body mass index (BMI), and weight gain. The size of the stone contributes significantly to the symptoms you’ll experience, along with the treatment you’ll receive, which will further be discussed.

After the formation of a kidney stone, the treatment is decided based on, most importantly, its size. The tube that moves urine from your kidney to your bladder is called the ureter. The average ureter has an internal diameter of just 3mm to 4mm. Stones that are smaller in size can pass easily through the ureter, with little to no discomfort.

The size of the stone can play a major role in determining the symptoms you’ll experience, and how severe they’ll be. Small stones generally exhibit less severe symptoms compared to larger stones. Additionally,  doctors can not plan the treatment unless they’re clear about the size of the stone.

Kidney stones 8 mm or larger usually require surgery or a medical procedure, as they rarely pass naturally through urine.

Yes. Stones smaller than 6 mm have a higher chance of passing naturally through urine with fluids and medication.

Yes. Kidney stones can grow over time, making treatment more complex and increasing the risk of kidney damage.
