Ectopic Kidney Stone

An ectopic kidney is one that has failed to ascend to its normal retroperitoneal position during fetal development. Ectopic kidneys can be located in the pelvis, iliac fossa, thorax, or crossed fused to the other side. Stones in ectopic kidneys are more common due to drainage abnormalities and urinary stasis associated with the abnormal anatomy. Treatment of stones in ectopic kidneys requires careful preoperative planning to account for the abnormal position, altered vasculature, and modified access routes for endoscopic or percutaneous procedures. Pristyn Care urologists are trained in managing stone disease in anatomically complex kidneys including all forms of ectopic kidney.

Symptoms

Ectopic kidney stones may cause the following symptoms:

  • Pelvic or lower abdominal pain rather than typical flank pain
  • Recurrent urinary tract infections
  • Blood in the urine
  • Nausea and vomiting
  • Difficulty emptying the bladder in pelvic ectopic kidney
  • Hydronephrosis detected on imaging
  • Incidental stone discovery on pelvic imaging

Are you going through any of these symptoms?

Causes

Kidney stones in ectopic kidneys develop due to:

  • Urinary stasis from abnormal drainage due to malrotation and unusual ureter course
  • Associated PUJ obstruction common in ectopic kidneys
  • Vesicoureteral reflux in some ectopic kidneys causing chronic infection
  • Metabolic risk factors for stone formation
  • Incomplete emptying of the collecting system due to gravitational disadvantage
  • Recurrent urinary tract infections promoting struvite stone formation

Types of Ectopic Kidney

Ectopic kidney positions relevant to stone management include:

  • Pelvic kidney – most common ectopic position, located in the pelvis
  • Iliac kidney – located at the level of the iliac vessels
  • Thoracic kidney – rare, located above the diaphragm
  • Crossed ectopic kidney – kidney crossed to the opposite side, often fused
  • Horseshoe kidney – lower pole fusion variant of ectopic kidney

Risk Factors

Risk factors for stone formation in ectopic kidneys include:

  • Abnormal drainage due to malrotated collecting system
  • Associated PUJ obstruction causing urinary stasis
  • Vesicoureteral reflux and recurrent infections
  • Metabolic conditions predisposing to stone formation
  • Low fluid intake in a kidney with impaired drainage
  • Delayed diagnosis of ectopic kidney allowing chronic stasis

Who is at Risk

Patients with a known ectopic kidney are at increased lifetime risk of stone formation due to structural drainage problems. Those with associated PUJ obstruction, vesicoureteral reflux, or metabolic stone disorders are at the highest risk and require regular surveillance.

Diagnosis

Diagnosis of stones in ectopic kidneys involves:

  • Non-contrast CT scan to confirm kidney position, stone location, and collecting system anatomy
  • CT urogram to assess drainage and identify obstruction
  • Ultrasound to evaluate hydronephrosis
  • DMSA or MAG-3 scan to assess differential kidney function
  • Urine culture to detect associated infection
  • 3D CT reconstruction to plan surgical or percutaneous access

Stone Treatment in Ectopic Kidney

Treatment of stones in ectopic kidneys depends on the stone size and the exact ectopic position. Ureteroscopy with laser lithotripsy is effective for most accessible stones. PCNL requires modified patient positioning and access planning based on the kidney location using CT guidance. ESWL has limited effectiveness due to difficulties with stone targeting in abnormal positions. Open or laparoscopic surgery is reserved for complex cases with associated obstruction.

Ectopic Kidney Stone Removal Procedure

Stone removal in ectopic kidneys is performed as follows:

  • Detailed preoperative CT and 3D planning to map the kidney position and optimal access
  • Ureteroscopy with flexible laser lithotripsy for accessible ureteral or renal stones
  • PCNL using modified patient positioning with ultrasound and fluoroscopic guidance
  • Laparoscopic assistance for PCNL access in deeply located pelvic kidneys
  • Simultaneous pyeloplasty if associated PUJ obstruction is present
  • A nephrostomy tube is placed for postoperative drainage
  • Post-procedure imaging to confirm stone clearance
  • Metabolic evaluation and prevention strategy initiated post-surgery

After Surgery Care

Post-operative care after stone surgery in an ectopic kidney includes:

  • Nephrostomy tube management and removal when output clears
  • Ureteral stent removal at 2 to 4 weeks if placed
  • Adequate hydration to support urinary drainage
  • Follow-up imaging to assess stone clearance and residual fragments
  • Metabolic workup to identify and treat stone-forming risk factors
  • Regular long-term surveillance imaging every 12 to 18 months

Complications of Ectopic Kidney Stone Surgery

Complications of stone surgery in ectopic kidneys include:

  • Injury to adjacent pelvic structures including iliac vessels or bowel
  • Bleeding from abnormal renal vasculature
  • Residual stone fragments from access difficulties
  • Urinary tract infection or sepsis
  • Failure to achieve stone-free status requiring repeat procedures
  • Damage to the ureter during retrograde access
  • Persistent obstruction from associated PUJ narrowing

Frequently Asked Questions

Why do ectopic kidneys develop stones more frequently?

Ectopic kidneys have abnormal drainage due to malrotation and unusual ureter course. This leads to urinary stasis, a major driver of stone formation. Associated PUJ obstruction and reflux further increase stone risk.

Can PCNL be performed on a pelvic ectopic kidney?

Yes, PCNL can be performed on pelvic ectopic kidneys by experienced urologists using modified patient positioning and CT-guided access planning. Laparoscopic assistance may be used to safely create the percutaneous access tract.

Is ESWL effective for stones in ectopic kidneys?

ESWL is generally less effective for ectopic kidney stones due to difficulty targeting the stone accurately and impaired drainage of stone fragments. Ureteroscopy or PCNL are usually more effective treatment options.

What happens if a stone in an ectopic kidney is left untreated?

Untreated stones in an ectopic kidney can cause progressive hydronephrosis, recurrent infections, and permanent loss of kidney function. Early treatment is important to preserve the function of the ectopic kidney and prevent complications.
