Horseshoe Kidney Stones

Horseshoe kidney is a congenital fusion anomaly in which the lower poles of both kidneys are joined across the midline by a bridge of parenchyma or fibrous tissue. This altered anatomy results in malrotation, abnormal collecting system position, and a higher ureter insertion, all of which predispose to urinary stasis and stone formation. Kidney stones develop in approximately 20 percent of patients with horseshoe kidney. Stone management in horseshoe kidneys is technically challenging due to altered anatomy, and standard approaches must be modified accordingly. PCNL, ureteroscopy, and ESWL are all used with technique adaptations. Pristyn Care urologists are experienced in managing stones in anatomically complex kidneys.

Symptoms

Horseshoe kidney with stones may cause the following symptoms:

  • Flank pain or abdominal pain lower than typical kidney stone pain
  • Recurrent urinary tract infections
  • Blood in the urine
  • Nausea and vomiting
  • Urinary tract infections that are difficult to clear
  • Hydronephrosis detected on imaging
  • Incidental discovery of stones on abdominal imaging

Are you going through any of these symptoms?

Causes

Kidney stones in horseshoe kidneys form due to:

  • Urinary stasis from abnormal collecting system anatomy and high ureteral insertion
  • Impaired drainage due to the fused lower poles and malrotation
  • Urinary tract infections associated with structural abnormality
  • Metabolic risk factors such as hypercalciuria or hypocitraturia
  • Associated PUJ obstruction in horseshoe kidneys increasing stone risk
  • Incomplete emptying of the collecting system promoting crystal deposition

Anatomy of Horseshoe Kidney

Key anatomical features of horseshoe kidney relevant to stone management include:

  • Lower pole fusion across the midline forming a U-shaped kidney
  • Medial rotation of the renal pelvis making standard access difficult
  • High ureteral insertion causing incomplete drainage
  • Abnormal calyceal orientation requiring modified PCNL access
  • Associated anomalies including PUJ obstruction in 30 percent of cases

Risk Factors

Risk factors for stone formation in horseshoe kidneys include:

  • Congenital urinary stasis from the abnormal anatomy
  • Associated PUJ obstruction causing further drainage impairment
  • Recurrent urinary tract infections
  • Metabolic disorders including hypercalciuria and hypocitraturia
  • Low fluid intake in a kidney with already poor drainage
  • Previous stone episodes in the horseshoe kidney

Who is at Risk

Patients with a known diagnosis of horseshoe kidney have a lifelong risk of stone formation. Those with associated PUJ obstruction, recurrent infections, or metabolic stone-forming conditions are at highest risk. Regular surveillance imaging is essential to detect stone formation early.

Diagnosis

Diagnosis of stones in horseshoe kidneys involves:

  • Non-contrast CT scan to identify stone location, size, and relation to altered anatomy
  • CT urogram to assess collecting system anatomy and drainage pattern
  • MAG-3 or DTPA diuretic renogram to assess for associated PUJ obstruction
  • Urine culture to detect associated infection
  • Metabolic workup to identify treatable risk factors for recurrence
  • 3D reconstruction imaging to plan surgical access routes

Stone Treatment in Horseshoe Kidney

ESWL has lower success rates in horseshoe kidneys due to impaired fragment drainage. PCNL using prone or modified access positions can effectively remove stones but requires careful planning due to altered calyceal anatomy. Ureteroscopy with flexible laser lithotripsy is effective for accessible stones. Open or laparoscopic surgery is reserved for complex cases with associated PUJ obstruction requiring simultaneous correction.

Stone Removal Procedure in Horseshoe Kidney

Stone removal in horseshoe kidney is performed as follows:

  • Preoperative CT and 3D planning to map calyceal anatomy and optimal access route
  • PCNL performed in prone or modified position using ultrasound and fluoroscopic guidance
  • Flexible nephroscopy to access stones in multiple calyces
  • Ureteroscopy with holmium laser for ureterally accessible stones
  • Simultaneous pyeloplasty if associated PUJ obstruction is present
  • A nephrostomy tube is placed for postoperative drainage
  • Post-procedure imaging to confirm stone clearance
  • Metabolic evaluation and long-term prevention plan

After Surgery Care

Post-operative care after stone surgery in horseshoe kidney includes:

  • Nephrostomy tube management and removal when output clears
  • Ureteral stent removal at 2 to 4 weeks if placed
  • Adequate hydration to support urinary drainage
  • Follow-up imaging to assess stone clearance and residual fragments
  • Metabolic workup to identify and treat stone-forming risk factors
  • Regular surveillance to detect new stone formation early

Complications of Stone Surgery in Horseshoe Kidney

Complications of stone surgery in horseshoe kidney include:

  • Bleeding during PCNL due to altered vascular anatomy
  • Injury to the isthmus or fused lower poles during surgical access
  • Residual stone fragments due to difficulty accessing all calyces
  • Urinary tract infection or sepsis
  • Damage to adjacent structures including bowel or vessels
  • Failure of stone clearance requiring repeat procedures
  • Persistent obstruction from associated PUJ narrowing

Frequently Asked Questions

Why are horseshoe kidney patients more prone to kidney stones?

Horseshoe kidney causes abnormal renal anatomy with urinary stasis, high ureteral insertion, and impaired drainage. These factors promote stone crystallization and accumulation. Associated PUJ obstruction further increases stone formation risk.

Can PCNL be done in a horseshoe kidney?

Yes, PCNL can be safely performed in horseshoe kidney by experienced urologists with careful preoperative planning using CT-guided 3D imaging to identify the safest access tract for stone removal.

Is ESWL effective for horseshoe kidney stones?

ESWL has lower success rates in horseshoe kidney due to impaired stone fragment drainage from the abnormal collecting system. PCNL or ureteroscopy with laser lithotripsy are generally more effective alternatives.

Does horseshoe kidney affect long-term kidney function?

Horseshoe kidney itself does not necessarily impair kidney function. However, recurrent stone disease, associated PUJ obstruction, and chronic infections can gradually reduce function if not properly managed with regular monitoring.
