Factors favouring laparoscopic pyelolithotomy:

  • Large single stone in the renal pelvis over 2 cm
  • Failed ESWL or PCNL attempts
  • Simultaneous UPJ obstruction requiring pyeloplasty
  • Prior open surgery making PCNL access difficult
  • Anomalous kidney anatomy limiting percutaneous access

Patients with large solitary stones in the renal pelvis who are not suitable for PCNL, those with simultaneous UPJ obstruction, and patients who prefer a minimally invasive alternative to open surgery are ideal candidates for laparoscopic pyelolithotomy.

Pre-operative evaluation includes:

  • Non-contrast CT KUB for stone location and size mapping
  • CT urogram for collecting system anatomy
  • Renal function tests and urine culture
  • Ultrasound for kidney dimensions and hydronephrosis
  • Metabolic stone workup for recurrence prevention

Treatment Options

Laparoscopic pyelolithotomy is performed via retroperitoneal or transperitoneal laparoscopic access. The renal pelvis is incised and stone is removed intact or in fragments. Simultaneous laparoscopic pyeloplasty for UPJ obstruction can be performed.

The Procedure

Laparoscopic pyelolithotomy involves:

  • General anesthesia with patient in lateral position
  • Port placement for laparoscopic retroperitoneal or transperitoneal access
  • Kidney and renal pelvis identification and dissection
  • Pyelotomy incision for stone access
  • Stone removal using laparoscopic forceps or suction
  • Irrigation and flexible scope check for residual fragments
  • Pyelotomy closure with intracorporeal suturing and drain placement
