Risk Factors

Risk factors for pyonephrosis:

  • Obstructing stone with delayed presentation
  • Diabetes mellitus and immunocompromised state
  • Prior urinary tract infections
  • Urinary tract structural abnormalities
  • Prior renal procedures or indwelling ureteral stents

Who is at Risk

Patients with obstructing ureteric or renal stones who develop urinary tract infections, diabetic patients, immunocompromised individuals, and elderly patients with delayed presentation are at highest risk for pyonephrosis.

Diagnosis

Urgent diagnostic workup includes:

  • Blood cultures and urine culture for organism identification
  • Serum creatinine, WBC count, and inflammatory markers (CRP)
  • Non-contrast CT KUB for stone location and kidney condition
  • Renal ultrasound to detect dilated system with echogenic debris
  • Sepsis workup including lactate level
  • ICU review for haemodynamically unstable patients

Treatment Options

Emergency percutaneous nephrostomy is the primary intervention, with bedside or ICU placement in critically ill patients. Intravenous broad-spectrum antibiotics are started immediately. Definitive stone removal via PCNL is performed 2 to 6 weeks later after complete infection clearance.

The Procedure

Emergency and staged management involves:

  • Emergency nephrostomy tube insertion under ultrasound guidance
  • Immediate IV broad-spectrum antibiotics covering gram-negative organisms
  • Purulent urine drainage and microbiological culture
  • ICU or HDU care for haemodynamically unstable patients
  • Nephrostomy tube maintained until urine clears and infection resolves
  • Staged PCNL via established nephrostomy tract 2 to 4 weeks later
  • Complete stone clearance with post-operative antibiotic continuation

After the Surgery

Post-operative care includes:

  • IV antibiotics transitioned to oral after culture sensitivity results
  • Daily creatinine and WBC monitoring post-drainage
  • Nephrostomy tube output monitoring and irrigation if required
  • CT KUB after stone clearance to confirm stone-free status
  • Long-term antibiotics for struvite stone prevention
  • Regular urine cultures every 3 months after treatment
