Who Needs Vein Stripping Surgery?

Vein stripping is recommended when varicose veins are large, tortuous, or associated with venous ulcers. It is also indicated when conservative treatments such as compression stockings have failed, when the patient has recurring superficial thrombophlebitis, or when the veins are not suitable for minimally invasive laser or radiofrequency procedures due to their size or anatomy.

Diagnosis

Diagnosis of varicose veins before surgery includes:

  • Clinical examination to assess vein extent, size, and symptoms
  • Doppler ultrasound to evaluate blood flow and detect valve incompetence
  • Duplex scan to map the venous anatomy and identify diseased segments
  • Trendelenburg test to assess the level of venous reflux
  • Photography to document the extent and distribution of varicose veins

Surgical Procedure

Vein stripping is performed under general, spinal, or epidural anesthesia. Small incisions are made at the groin and ankle. A wire or stripper tool is passed through the vein and used to pull it out. Ligation is performed at the saphenofemoral junction. Compression bandages are applied after the procedure. Surgery typically takes 60 to 90 minutes.

Vein Stripping Procedure Steps

The vein stripping procedure involves:

  • Administering anesthesia and positioning the patient
  • Making small incisions at the groin and above the ankle
  • Identifying and ligating the saphenofemoral junction at the groin
  • Inserting a stripper wire through the vein from groin to ankle
  • Attaching the vein to the stripper and pulling it out through the lower incision
  • Removing additional varicose branches through small stab incisions (phlebectomy)
  • Closing all incisions and applying firm compression bandages

After the Surgery

Recovery after vein stripping takes 2 to 4 weeks. Patients typically go home the same day or after one night. Compression stockings are worn for 2 to 6 weeks. Walking is encouraged from the first day to promote circulation. Avoid standing for long periods, heavy lifting, and strenuous exercise for 4 weeks. Bruising and swelling along the stripped vein gradually resolves over 2 to 4 weeks.
