What is lipoma in breast​?

A lipoma in breast refers to a benign, non-cancerous breast tumor. Lipomas are fatty tumors that can develop anywhere in the body. They don’t hurt and don’t increase the risk of developing cancer. Unlike other types of tumors, lipomas rarely cause any symptoms.

Breast lipomas develop from mesenchymal cells, which are soft tissue cells. Essential tissues like bones, muscles, nerves, joints, and adipose(fatty) tissue are formed and maintained by these cells. Breast lipomas are slow-growing tumors that are located just beneath the skin. They consist of mature fat cells contained in thin, fibrous capsules rather than breast tissue.

Lipoma in breast symptoms​

Lipomas in breast are the benign tumors and often do not cause symptoms. But if a lipoma affects adjacent joints, blood arteries, or nerves, it could be painful. Breast lipomas can develop in clusters or one at a time.

  • They usually weigh only a few grams and are less than a centimeter. Sometimes these fatty tumors can also grow larger.
  • Those that measure more than 10 cm and weigh more than 500g are considered giant breast lipomas. They can also form a bulge that is visible on the surface of the breast
  • Large lipomas on the breast that can be felt beneath the skin are usually doughy and soft to the touch, and they can move around easily. 
  • They don’t pull or bind the skin over it. Instead, if you gently push the mass, it will slide out from under your fingers.

Types

A benign breast tumor is a non-cancerous growth in the breast. The following are the types of benign breast tumors.

  • Cyst

It refers to a fluid-filled sac inside the breast. They are benign(non-cancerous), and you can have one or multiple cysts in the breast. Unless the cyst is large and uncomfortable, they don’t require treatment.

  • Fibroadenoma

Fibroadenoma refers to a solid non-cancerous lump in the breast. Although it can develop at any age in anyone who has periods, it usually develops between the ages of 15-35. They don’t cause pain and can feel firm, smooth, and rubbery. Fibroadenomas are round in shape and can feel like a pea in the breast. Sometimes they may also feel flat like a coin.

  • Intraductal papilloma

Non-cancerous breast tumors in your milk ducts are referred to as intraductal papillomas. “Papillomas” are growths that resemble warts and are covered in epithelial cells. Your body’s outside and interior surfaces are covered in epithelial cells. “Intraductal” simply indicates that the tumors are located within your ductal milk ducts.

  • Hamartoma

A hamartoma growth resembles a tumor and can develop anywhere in the body. Most hamartomas are not dangerous. However, if they are large enough to harm adjacent tissue, they may cause problems.

  • Neurofibroma

A noncancerous tumor that resembles a soft lump on your skin is called a neurofibroma. These tumors develop on nerves. Some appear just beneath the surface of your skin, while others develop more deeply within your body.

  • Hematoma

When the blood cann’t flow or drain out, it collects and fills a space inside your body, making a closed wound. It refers to the development of hematomas. Minor hematomas are common and can develop anywhere in your body. When they grow larger, they begin to press on the surrounding tissues.

Causes

Several studies have indicated that trauma or injury to the breast leads to an increased production of fatty tumors and fat cells. A family history of lipomas increases the risk of developing this condition.

The following are a few common causes.

  • Multiple hereditary lipomatosis

Multiple hereditary lipomatosis is a rare inherited medical condition. It is characterized by the presence of numerous lipomas in the arms, legs, or abdominal area.

  • Gardner Syndrome

It is a genetic condition involving polyps growth inside and outside of the colon.

  • Adiposis dolorosa

It is a medical condition where fatty tissue folds apply pressure to the nerves, causing pain.

  • Madelung disease

It is a rare disorder and characterizes the growths in the upper part of the body, such as, neck, arms, shoulders, and chest.

Diagnosis

  • Physical examination

To diagnose a lipoma on the breast, your doctor conducts a physical examination, including a breast exam.

  • Mammogram

A routine mammogram helps diagnose a lipoma on the breast that you don’t feel otherwise. The lipoma mass can be clearly seen on the mammogram of your breasts. A mammogram is a low-dose X-ray and is helpful in screening and diagnosing breast cancer.

  • Ultrasound

It uses sound waves to produce an image of the breast tissue. Ultrasound helps to distinguish between a cancerous(malignant) and a non-cancerous(benign) lipoma.

  • Biopsy

Biopsy involves taking a tiny sample of fluid or tissue from the breast lump and examining it under a microscope. A biopsy also helps in diagnosing breast cancer.

When should you see a doctor?

You should consult a doctor whenever you feel a new breast lump or signs of breast cancer. A family history of breast cancer plays an important role, too.

  • Nipple discharge
  • Skin changes on your breast, such as the color of the skin, etc.
  • Irritation on the breast skin or nipple
  • Flaky skin or redness in the nipple area
  • Pulling in of the nipple area
  • Swelling on the breast
  • The lump has changed its size and hasn’t gone away after 4 to 6 weeks

Treatment

Most lipomas on the breast are harmless and do not cause pain or other symptoms. At first, your doctor may choose to observe the lipoma on the breast to be sure that they do not grow too large. They will observe the lipoma for some time to understand if it causes symptoms or other issues or not. The doctor may recommend the Lipoma treatment if he notices the following:-

  • Lipomas are larger than 10 cm
  • Rapidly growing lipomas
  • Connected with underlying medical conditions
  • Lipomas are causing pain
  • Lipomas in precarious sites, such as close to joints or nerves, or deep tissues

If it causes a problem or if you do not like its appearance, you can get it removed surgically.

Surgery

It involves removing the entire lipoma mass on the breast, along with its fibrous capsule, to reduce the risk of recurrence. 

  • Liposuction

The procedure uses a needle and a syringe to remove the lump. Liposuction is used to suction out the clusters of breast lipoma or single tumors that are either medium-sized, 4-10 cm, or larger. The procedure includes minimal pain, fewer scars, and reduced risks and complications. 

  • Shrinking

Lipomas are injected with medications before the surgery to shrink them. It helps in breaking down the fat tissues from the tumors.

What should you do before/during the surgery?

Before

The doctor’s instructions may include the following.

  • Before the procedure, wash the area properly using an anti-bacterial agent
  • Stop taking the ACE inhibitors or blood thinners whenever asked by the doctor
  • Avoid taking medications without consulting your doctor

During

  • To numb the surgical site, a local anaesthetic is injected around the lump
  • General anaesthesia is given if the lipoma is large, more than 10 cm
  • To remove the lipoma, the doctor will make a cut
  • Lipoma is then removed, and the wound is stitched
  • The surgery may take somewhere between 30 minutes to one hour.

The incision length is usually about the width of the lipoma. The doctor may also use the squeeze technique, which leads to minimal scars. However, this is not effective for large lipomas.

Recovery

  • In most cases, you are allowed to go home the same day.
  • Depending on the type, the stitches can either dissolve by themselves or need to be removed.
  • You may experience discomfort for a few days around the surgical site. The
  • doctor may prescribe painkillers to address the pain.
  • Slight swelling is normal after the surgery. You can apply ice over the bandages to manage the same.
  • Follow your doctor’s advice if you want to resume daily activities.

Risks and complications

Generally, it is considered a safe procedure, but every surgery carries some risks. Sometimes you may develop an infection or bleed. Consult your surgeon if the stitches break or you develop symptoms indicating an infection, such as;
