What is a Grade 1 Varicocele?

A varicocele is the collection of inflamed or swollen veins inside the scrotum, i.e., they are varicose veins inside the scrotal sac. It generally occurs due to abnormality in the blood flow in the scrotal veins, which leads to the pooling of blood in the veins and inflammation.

In this stage, the varicocele is not easily visible. Patients might feel slight swelling in the scrotal region. They also start experiencing mild symptoms such as discomfort and soreness. They can be diagnosed during a physical exam through a Valsalva maneuver.

It can also result in a reduced proportion of sperm in the patient’s semen. This is because it blocks the blood flow in the testes, which impedes reproductive function and can even lead to permanent damage to the testicles, also known as testicular atrophy.

When is Grade 1 Varicocele Treatment Required?

Grade 1 varicocele usually doesn’t cause any noticeable symptoms. In fact, most patients with Grade 1 varicocele are not even aware they have a varicocele. However, its treatment may be required if:

  • The patient has a low sperm count or any other sperm-related problems
  • The varicocele is causing severe pain and discomfort to the patient
  • The patient and their partner are trying to have a baby but have unexplained infertility, but the patient has a varicocele.

If left untreated, the varicocele can eventually lead to permanent testicular atrophy and infertility for the patient.

How is Grade 1 Varicocele Treated?

A grade 1 varicocele is usually managed medically because it rarely causes any noticeable symptoms. Grade 1 varicocele symptoms are managed by applying ice packs to reduce swelling, using over-the-counter pain medications for pain management, and wearing tight-fitting underwear to prevent irregular blood flow and pooling.

However, in case the symptoms become problematic, the varicocele is removed surgically. The surgical removal of a varicocele is known as varicocelectomy. The most common and best procedures for varicocele removal are:

  • Open varicocelectomy: It is an open surgery in which the surgeon makes an incision directly on the scrotum, groin or lower abdomen to find and tie off the varicose blood vessels. It is usually not preferred as it requires a long recovery period and causes scarring.
  • Microscopic varicocelectomy: Microscopic varicocelectomy is performed by creating a microscopic (less than 1 mm.) incision on the scrotum. The surgeon then examines the veins under a microscope before ablating the veins. Since the incision is so small, it causes minimal scarring and provides a quicker recovery period.
  • Laparoscopic varicocelectomy: In this procedure, the surgeons make a tiny incision on the belly to insert the laparoscope and surgical instruments. The laparoscope helps the surgeon visualize and treat all varicose veins separately. Since it is also a minimally invasive procedure, it is also preferred over open varicocelectomy.
  • Percutaneous embolization: During embolization, instead of removing the varicocele, the surgeon instead blocks the blood flow to the varicose veins. In this procedure, the surgeon inserts a tube in the veins through the groin region. The surgeon inserts a coil or solution that scars the vein and blocks the opening of the varicose veins.

How Can One Prepare for Varicocele Surgery?

If the patient needs surgery, they can prepare for the same via:

  • Before the surgery, let your surgeon know if you’re taking any medications or supplements. 
  • If you are taking any blood thinners, you may have to stop them at least a few days before the surgery to lower the risk of bleeding during the surgery.
  • You may also be asked to avoid any food or drinks for at least 8 to 12 hours before the surgery. 
  • After the surgery, you will not be able to drive yourself home, so you should arrange for transportation once you are done with the surgery.

What to Expect After a Grade 1 Varicocele Treatment?

Generally, a Grade 1 varicocele is managed medically, but in case surgery is required, the patient will be hospitalized for 2-3 hours in a recovery room. After this, patients can go home on the same day.

Most men can start working within a couple of days of the surgery, especially if they have desk jobs. There may be slight postoperative pain and swelling in the groin for 3 to 4 weeks, but that can be easily resolved through over-the-counter pain medication. Complete recovery may take at least 3-6 weeks.

  • Take care of your incisions. Keep them clean and dry, and change your bandages as directed. If you notice any signs of infection, such as redness, discharge, fever, etc., then contact your doctor immediately.
  • Do not perform any strenuous activities or sports for at least 2-3 weeks after the surgery.
  • Take your medicines as prescribed. Apply an ice pack to your scrotum for at least 10 minutes several times daily to keep the postoperative swelling down.
  • Do not engage in sexual intercourse for at least 2 weeks after the surgery.
  • Do not swim, take a bath or perform any activity involving immersing your scrotum in water until the incisions have closed.
  • Manage your diet to avoid constipation and straining while defecating. Eat a balanced diet that is rich in fiber, and hydrate yourself properly.

Although varicocele removal surgery is a relatively safe procedure, just like other surgeries, it can be associated with some risks and complications:

  • Hemorrhage
  • Infection
  • Blood clot formation in leg veins (Deep vein thrombosis)

  • Difficulty in emptying the bladder/Bladder obstruction
