What is Chronic Venous Insufficiency?

Chronic Venous Insufficiency is a type of venous disease and is caused by damage to the veins in your legs. The veins loose their ability to control blood flow as they should, and the condition makes it harder for the blood in your legs to return to the heart. Consequently, the blood pools in the veins of your legs, raising the pressure in those veins. 

The damage to your leg veins may lead to the development of Chronic Venous Insufficiency. These veins include:-

  • Large veins that travel through your muscles deep within the body are known as deep veins
  • Superficial veins close to the skin’s surface
  • Perforating veins, which connect the deep and superficial veins

Causes of Venous Insufficiency

Chronic Venous Insufficiency develops when the vein valves in your legs don’t work properly. Your leg contains valves that help in maintaining the flow of blood in the right direction, which is toward your heart. A broken valve is unable to close properly. And due to gravity, the blood finds it difficult to move upward toward your heart. Rather, it flows backward, causing a condition called venous reflux.

Various causes of valve malfunction are given below. 

  • Congenital causes. Vein abnormalities in your legs that are present from birth are known as congenital causes. Some people, for instance, are born without valves in their veins in their legs.
  • Primary causes. It refers to the factors that cause changes to your leg veins, affecting their normal functioning. For instance, your vein can enlarge too much, preventing the valve from closing.
  • Secondary causes. These are the additional medical conditions that harm the veins in your legs. The most common secondary cause is deep vein thrombosis (DVT), wherein the scar tissue left behind by the thrombus (blood clot) harms your valve.

Stages of Chronic Venous Insufficiency

  • C0

There are no visible signs of CVI in stage C0. At this stage, you won’t have any swelling or noticeable veins. Your doctor will probably advise you to adopt a healthier diet and engage in more exercise because losing weight can reduce the progression of CVI.

  • C1

You will begin to notice symptoms of CVI in stage C1. You will also notice reticular veins, which are visible veins that range in diameter from 1 to 3 millimeters (mm). Additionally, you can have visible blood vessels that are less than 1 mm in diameter, known as telangiectasias or spider veins.

While telangiectasias show up as tiny red lines beneath your skin, reticular veins are usually blue or purple. Your blood arteries and veins will be visible at stage C1, but they won’t be protruding.

  • C2

You will begin to notice varicose veins at this stage. Because varicose veins are swollen, you can feel them sticking out from under your skin. Stage C2r is the substage of stage C2. This stage pertains to varicose veins that keep returning despite therapy.

At this stage, doctors may advise you to wear compression clothing and exercise frequently. Varicose veins can also be corrected with treatments. The doctor may administer a sclerotherapy injection to cause your veins to scar down. Another method for treating varicose veins is endovenous ablation, which employs a laser to seal them off.

  • C3

At this stage, you will begin to experience edema. Edema is primarily caused by swelling in your legs. You will not experience any changes to the skin at this stage. For relief, you might be advised to wear compression garments and seek treatment for varicose veins, if needed. 

  • C4

You will begin experiencing changes to your skin along with edema at this stage. This stage is subdivided into three:-

  • C4a. The skin might change, and you might have red and itchy skin with small brown or grey markings on your skin. 
  • C4b. The skin may change its texture and can harden or thicken. You may also notice some scar marks along your legs. 
  • C4c. The veins and blood vessels around your ankles will be visible. It’s called corona phlebectatica 

Along with the treatment for swelling and varicose veins, the doctor may suggest some skin care to treat the skin changes.

  • C5

At this stage, you will experience open wounds, known as ulcers, on your legs. The doctor would suggest some medicated creams and dressings to help the ulcers heal. 

The substage of C5 says that it is not the first time the ulcers have formed, but they have come back despite treatment.

Diagnosis for Venous Insufficiency

A physical exam and an ultrasound imaging are done to diagnose the Chronic Venous Insufficiency.

  • Examine your legs: Your doctor examines the signs and symptoms of Chronic Venous Insufficiency, such as changes in skin color, ulcers, etc. 
  • Doppler ultrasound: Your doctor creates an image of your veins using sound waves to diagnose the damaged areas.

Chronic Venous Insufficiency Treatment Options

Medications

  • Antibiotics. These help to treat ulcers or skin infections caused by CVI. The underlying illness is not treated by these medicines.
  • Anticoagulants. “Blood thinners,” sometimes known as anticoagulants, are used to treat blood clots and prevent them from recurring.
  • An Unna boot is a medicated wrap. This wrap creates a semi-rigid bandage by combining multilayer compression with a wound cover made of zinc oxide gel.

 

Non-surgical Treatments

  • Sclerotherapy: Your doctor will inject a liquid or foam solution into your varicose vein or spider vein. As a result, the vein collapses or vanishes.
  • Endovenous thermal ablation: The method is used to target large veins. It generates extreme heat using a laser or high-frequency radio waves. In order to minimize bleeding or bruising, this heat seals off the damaged vein while leaving it intact.

 

Surgical Treatment

  • Ligation and stripping: These two operations are usually carried out together. In vein ligation, the problematic veins are cut and tied off. Stripping involves making two tiny incisions and surgically removing bigger veins.
  • Microincision or ambulatory phlebectomy. It is a minimally invasive treatment. It targets varicose veins that are close to the skin’s surface. Your healthcare provider makes tiny cuts or punctures your veins with a needle. The problematic veins are then removed using a phlebectomy hook.
  • Subfascial endoscopic perforator surgery. It is a minimally invasive technique. It targets the veins that perforate above your ankle. To prevent blood from passing through damaged veins, your healthcare practitioner will use a clip to block off the damaged veins. SEPS helps in the healing and recurrence prevention of ulcers.
