  • What is a Grade 2 Varicocele?

    Varicoceles are varicose veins present in the scrotal sac. They are a collection of inflamed or swollen veins that are formed due to abnormality in the blood flow in the scrotal veins. This abnormal blood flow causes the pooling of blood in the veins. Eventually, these veins turn into varicose veins.

    Grade 2 varicoceles start becoming visible and palpable for the patient. The patient can feel swelling in the scrotal region and, in some cases, can see it naturally. They can be diagnosed during a physical exam through a Valsalva maneuver.

    A Grade 2 varicocele can cause significant pain for the patient. Moreover, at this stage, the patient’s fertility is also affected noticeably. It can lead to reduced sperm proportion in the patient’s semen. In severe cases, it can even lead to permanent damage to the testicles, also known as testicular atrophy.

    When is Grade 2 Varicocele Treatment Required?

    A grade 2 varicocele is when the varicocele actually starts becoming noticeable and starts causing severe symptoms. Though most patients prefer conservative management of symptoms at this time, surgical treatment may be necessary in severe cases.

    Surgical treatment for grade 2 varicocele may be required in the following cases:

    • The patient has a low sperm count or poor quality sperm in sperm, leading to infertility
    • The varicocele is causing severe pain and discomfort to the patient
    • There is a chance of varicocele getting worse and causing testicular atrophy in the patient.

    If left untreated, varicocele can lead to low testosterone and its associated complications like metabolic syndrome, diabetes, and osteoporosis.

    How is Grade 2 Varicocele Treated?

    If the patient has mild to moderate pain, or if their fertility isn’t being impacted, then the patient may need only conservative management for their symptoms. Conservative symptomatic management includes applying ice packs to reduce swelling, using over-the-counter pain medications for pain management, and wearing tight-fitting underwear to prevent irregular blood flow and pooling.

    In cases where varicocele removal is necessary, the patient can undergo the following procedures:

    • Percutaneous embolization: It is a non-surgical procedure during which the surgeon blocks the blood flow to the varicose veins to block blood flow through them. For this procedure, the surgeon inserts a coil or solution through the groin region that scars the vein and blocks the opening of the varicose veins.
    • Open varicocelectomy: For open varicocelectomy, the surgeon creates an incision directly on the scrotum, groin, or lower abdomen. They directly locate and tie off the varicose blood vessels through the incision. It has a long recovery period and causes scarring, so nowadays, it is generally not preferred for varicocele treatment.
    • Microscopic varicocelectomy: It is a minimally invasive procedure that is performed by creating a microscopic (less than 1 mm.) incision on the scrotum. Then, the surgeon uses a microscope to locate and remove the veins. Since the incision is so small, it causes minimal scarring and provides a quicker recovery period.
    • Laparoscopic varicocelectomy: For laparoscopic surgery, the surgeon uses a laparoscope to visualize and treat all varicose veins separately by making a tiny incision on the belly to insert the laparoscope and other surgical instruments. Since it is also a minimally invasive procedure, it is also preferred over open varicocelectomy.

    How Can One Prepare for Varicocele Surgery?

    For conservative treatment, you should consult with your vascular doctor about the precautions and preparations you should take. Some common precautions you may need to take before surgical treatment are:

    • Inform your surgeon about your complete medical history, including any medications or supplements you are taking.
    • If you have any blood-related disorders for which you are taking any blood thinners, you may have to stop them at least a few days before the surgery.
    • If the surgery is to be performed under general anesthesia, you should avoid food or drinks for at least 8 to 12 hours before the surgery. 
    • You may need some assistance for driving after the surgery to take you home, so arrange for someone to drive you.

    What to Expect After a Grade 2 Varicocele Treatment?

    A grade 2 varicocele can be managed medically, but if the pain is too much or if the patient’s fertility is being impacted, they may need surgical intervention. The recovery period is vastly different in both cases. 

    For conservative treatment, the varicocele pain can be managed. However, with surgical treatment, the patient will be hospitalized for 2-3 hours in a recovery room. After this, patients will be discharged on the same day.

    The patient can return to work within a few days of the surgery, especially if they have a desk job. They might have slight postoperative pain and swelling in the groin area for 3 to 4 weeks. This pain and discomfort can be managed through over-the-counter pain medication. Complete recovery after minimally invasive varicocelectomy may take at least 3-6 weeks.

    • Keep your incisions clean and dry. Change your bandages as directed by your surgeon. Do not swim, take a bath or perform any activity involving immersing your scrotum in water until the incisions have closed.
    • Avoid all strenuous activities or sports for at least 2-3 weeks after the surgery.

    Minimally invasive varicocelectomy is normally very safe. However, occasionally there may be complications during or after the surgery. Some potential risks of varicocelectomy are:

    • Blood clot formation, i.e., deep vein thrombosis, which can lead to pulmonary embolism
    • Hydrocele formation
    • Bladder obstruction due to scarring
