Varicocele Grading

Varicocele is graded from 1 to 3. Grade 1 is only detectable during Valsalva maneuver. Grade 2 is palpable without Valsalva. Grade 3 is visibly enlarged. Laparoscopic surgery is typically recommended for grades 2 and 3 varicocele and for grade 1 when associated with infertility or testicular atrophy. Bilateral varicocele affecting both sides may also be treated laparoscopically.

Who Needs Laparoscopic Surgery?

Laparoscopic varicocelectomy is recommended for men with clinical grade 2 or 3 varicocele, men with varicocele and infertility, adolescents with testicular growth arrest, and patients with chronic scrotal discomfort unresponsive to conservative management. It is also preferred for bilateral varicocele as both sides can be treated simultaneously through the same set of incisions.

Diagnosis

Diagnosis of varicocele requiring laparoscopic surgery includes:

  • Clinical examination in standing position with Valsalva maneuver
  • Scrotal Doppler ultrasound to confirm venous dilation and reflux
  • Semen analysis to assess sperm count, motility, and morphology
  • Hormonal evaluation including testosterone, FSH, and LH levels
  • Testicular volume measurement to detect atrophy
  • Abdominal imaging to rule out secondary varicocele caused by abdominal pathology

Surgical Procedure

Laparoscopic varicocelectomy is performed under general anesthesia. The surgeon makes 2 to 3 small incisions in the abdomen and inserts a laparoscope and instruments. Carbon dioxide gas is used to inflate the abdomen for visibility. The internal spermatic veins are identified at the level of the internal inguinal ring and ligated using clips. The procedure takes 30 to 60 minutes.

Laparoscopic Varicocele Steps

The laparoscopic varicocelectomy procedure involves:

  • Administration of general anesthesia and patient positioning
  • Creating 2 to 3 small incisions in the abdomen for port placement
  • Inflating the abdomen with carbon dioxide gas for visualization
  • Inserting the laparoscope and identifying the internal spermatic vein
  • Isolating the dilated veins while preserving the testicular artery and lymphatics
  • Clipping or ligating the identified veins to stop blood reflux
  • Deflating the abdomen and removing the instruments
  • Closing the small incisions with sutures or skin glue

After the Surgery

Recovery after laparoscopic varicocelectomy is quick. Most patients go home the same day or after one night. Mild abdominal discomfort and bloating from the gas may occur for 1 to 2 days. Light activities can be resumed within a week. Strenuous exercise and sexual intercourse should be avoided for 2 to 3 weeks. Semen analysis is repeated 3 to 6 months after surgery.

Potential complications of laparoscopic varicocelectomy include:

  • Hydrocele due to damage to lymphatic vessels
  • Recurrence of varicocele if veins are incompletely ligated
